Domar (Pinazepam) is a moderate anxiolytic. The pharmacological activities of Domar are characterized by a marked anxiolytic effect acting on the limbic system, amygdala, and especially the hippocampus, exerting a specific and selective action on emotional control. It has a good muscle relaxant effect without any depressant action on the spinal cord. Domar also has an excellent anti-convulsant effect. The therapeutic index of Domar is high and its therapeutic effects appear fast and are long lasting. Research into the pharmacokinetics of Domar in man has shown that it has a half-life of about 12 hours. This offers decisive therapeutic advantages as the patient enjoys life free from drowsiness with one capsule twice daily.
Domar is indicated in the treatment of anxiety states, psychoneurosis, tension, irritability, agitation, depressive illness (specially with a reactive or neurotic basis), depression in elderly people, that is, all those forms in which there may be anxiety of psychiatric significance. It is indicated in disease with a psychosomatic factor (respiratory disturbance, digestive symptoms, insomnia from anxiety, cardiocirculatory changes, neurocirculatory asthenia, pseudo-anginal pain in the chest). It is also indicated in chronic alcoholism and during withdrawal from its use. It can be used as a coadjuvant with other drugs in epilepsy.
A suitable dose in most cases is 10 mg a day, divided into two doses 12 hours apart or one capsule of 5 mg twice daily (b.d.) morning and evening. At this dosage the patient is free from drowsiness and his reaction time and concentration ability are not affected. In severe anxiety states the dosage may be increased to 20 mg a day divided into 2 capsules of 5 mg b.d. mornings and evenings.
Although the effect of Domar on neuromuscular control is much less evident compared to other benzodiazepines it is not advisable to use it in myasthenia gravis. Personal hypersensitivity to pinazepam discovered on taking the drug.
One must keep in mind that psychotropic drugs reduce tolerance to alcohol and this must be avoided during treatment with Domar. In general, patients treated with Domar should be aware of the possible effect of the drug on their ability to react (for example, driving a motor car and operating machines needing careful attention). This may occur especially if the dosage is high or at the commencement of treatment. Predisposed subjects, if administered very high dosage of Domar for long periods, can develop a tendency to dependence. With prolonged treatment in high dosage one should periodically examine the blood pressure, blood count and liver function tests.

It should not be prescribed in the first three months of pregnancy. In later stages of pregnancy it should be prescribed only if absolutely necessary and then only under careful medical monitoring. It should be used with caution in nursing mothers.
Safety and tolerance of Domar in therapeutic dosage (10 mg per day) is good. At the start of treatment and when large doses are administered one can occasionally see a more marked sedative action or slight somnolence. These effects disappear spontaneously during treatment or on adjusting the dosage. When administering benzodiazepines one has observed, although rarely, confusion, ataxia, skin rashes, digestive disturbance. Rarely one may encounter bone marrow depression and liver damage, these being usually reversible.
Association with other tranquilizers needs special care and vigilance on the part of the doctor so as to avoid undesirable and unexpected effects of interaction.
