Paracetamol
Indications/Uses
Relief of minor aches & pains due to the common cold, flu, sore throat, teething, headache, earache, post-immunization reactions, & toothache (for syr only). Fever reduction.
Dosage/Direction for Use
Max: Not to exceed 5 doses in 24 hr. Duration: Not to be used continuously for >5 days at a time. 120 mg/5 mL syr Childn 7-12 yr 10-15 mL every 4-6 hr, 3-6 yr 5-10 mL every 4-6 hr, 1-2 yr 5 mL every 4-6 hr. 250 mg/5 mL syr Childn 7-12 yr 5-7.5 mL every 4-6 hr, 3-6 yr 2.5-5 mL every 4-6 hr, 1-2 yr 2.5 mL every 4-6 hr. 100 mg/mL oral drops Childn 1-2 yr 1-1.2 mL every 4-6 hr. Infant 7 mth-<1 yr 1 mL every 4-6 hr, 4-6 mth 0.6-1 mL every 4-6 hr, 0-3 mth 0.3-0.6 mL every 4-6 hr.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Patients receiving other medicines that contain paracetamol.
Special Precautions
Discontinue use if symptoms of allergic reaction (eg, skin redness, blisters, or rash) occur; symptoms worsen or do not improve; new symptoms or any undesirable effect occur; redness or swelling is present; fever gets worse or lasts >3 days; pain gets worse or lasts >5 days in childn <12 yr & 10 days in adults; patient had unexpected bruising or bleeding, & had more infections than usual; sore throat is severe, persists for >2 days, is accompanied or followed by fever, headache, rash, nausea, or vomiting. Severe liver damage may occur if childn take >5 doses in 24 hr, or adult or child ≥12 yr take >4 g in 24 hr; in concomitant use w/ other medicines containing paracetamol. Patients w/ kidney or liver problems (including alcoholic liver disease); G6PD deficiency; whose illness whose cause is not clear or is accompanied by rash, breathing difficulties, diarrhea or excessive tiredness or lethargy; who are severely malnourished, anorexic, underwt, or excessively drink alcohol. Concomitant use w/ other painkillers or any other medicines.
Adverse Reactions
Anemia, changes in the number of WBC & platelets eg, agranulocytosis, leukopenia, methemoglobinemia, neutropenia, pancytopenia, thrombocytopenia; thrombocytopenic purpura; allergic reactions which may cause difficulty swallowing, unexplained wheezing, shortness of breath or difficulty in breathing, skin rash, hives, or angioedema, proneness to infections, swelling around the eyes; hyperchloremia, hyperuricemia, loss of appetite, metabolic acidosis, sudden wt loss; headache; unusual bleeding or bruising; resp distress, runny nose, laryngeal edema; mouth ulcers, nausea, sore throat, vomiting; fixed eruption, itchy rash, severe skin reactions (ie, SJS, TEN, & acute generalized exanthematous pustulosis); chronic hepatic necrosis in prolonged use, liver dysfunction & injury, yellowing of eyes & skin; kidney toxicity, papillary necrosis in prolonged use; fatigue or unusual tiredness; decreased serum bicarbonate, Ca & Na; increases in serum alkaline phosphatase, glucose & transaminase levels.
Drug Interactions
Risk of toxic effects to the kidneys & eventually of kidney failure may be increased in prolonged use w/ other painkillers (eg, NSAIDs, aspirin). Risk of paracetamol-induced liver toxicity may be increased w/ chronic intake of alcohol. The ability of the body to metabolize large doses of paracetamol may be diminished w/ acute intake of alcohol. Absorption may be increased by metoclopramide or domperidone. Reduced absorption w/ cholestyramine, narcotic pain relievers, & some antidepressants that slow down gastric emptying. Efficacy may be reduced & susceptibility to the harmful effects to the liver may be increased w/ medicines which stimulate the enzymes responsible for the metabolic activation of paracetamol eg, medicines for convulsion (eg, phenobarb), depression (eg, TCAs), & OCs. May increase the risk of severe hypothermia w/ phenothiazines. Increased exposure to chloramphenicol. Elimination may be affected w/ probenecid. May increase risk of bleeding w/ warfarin.
