Per 4.5 g vial Piperacillin 4 g, tazobactam 500 mg
Indications/Uses
Complicated & uncomplicated UTI; lower resp tract, intra-abdominal, skin & skin structure, gynaecological, bone & joint, polymicrobic (aerobic & anaerobic) infections; bacterial septicaemia. In combination w/ aminoglycoside for bacterial infections in neutropenic patients.
Dosage/Direction for Use
IV Slow infusion over 20-30 min or slow inj over at least 3-5 min. Adult & childn ≥12 yr w/ normal renal function 4 g piperacillin/0.5 g tazobactam every 8 hr for 7-10 days. Nosocomial pneumonia Max dose: 2.25 g every 8 hr for 7-14 days in patient on hemodialysis. Renal insufficiency w/ CrCl 20-40 mL/min 2.25 g every 6 hr, <20 mL/min 2.25 g every 8 hr. Patient on hemodialysis Max dose: 2.25 g every 12 hr w/ additional dose of 0.75 g piperacillin/tazobactam following each dialysis period. Appendicitis &/or peritonitis Ped patient weighing >40 kg w/ normal renal function Adult dose, ≥9 mth weighing up to 40 kg w/ normal renal function 100 mg piperacillin/12.5 mg tazobactam/kg every 8 hr, 2-8 mth 80 mg piperacillin/10 mg tazobactam/kg every 8 hr.
Contraindications
Hypersensitivity to penicillins & cephalosporins or β-lactamase inhibitors.
Special Precautions
Discontinue if allergic reactions; bleeding manifestations occur. Not indicated for viral infections. Previous hypersensitivity to penicillins, cephalosporins & other multiple allergens; anaphylactic/anaphylactoid reactions including shock. Pseudomembranous & antibiotic-associated colitis. Possibility of superinfections; hypokalemia. Neuromuscular excitability or convulsion. Increased incidence of fever & rash in cystic fibrosis patients; risk of drug-resistant bacteria development in absence of proven or strongly suspected bacterial infection or prophylactic indication. Leukopenia & neutropenia during prolonged therapy. Patients requiring restricted salt intake; receiving concomitant medications lowering K levels; w/ CrCl ≤40 mL/min & on dialysis. Periodically perform electrolyte determinations in patient w/ low K reserves; assess hematopoietic function in prolonged therapy ie, ≥21 days. Renal insufficiency & hemodialysis patients. Pregnancy & lactation. Ped patients <2 mth; w/ impaired renal function. Elderly >65 yr w/ renal insufficiency.
Adverse Reactions
Diarrhea, nausea, vomiting; rash. SJS, TEN.
Drug Interactions
Substantial inactivation of aminoglycoside. Longer t½ & lowered renal clearance w/ probenecid. Concomitant use w/ high dose heparin, oral anticoagulants or other drugs affecting blood coagulation system or thrombocyte function. Prolonged neuromuscular blockade of vecuronium or any non-depolarizing muscle relaxants. Reduced MTX clearance. False +ve reaction for glucose urine test.
