Indications/Uses
Dosage/Direction for Use
Initially 7.5 mg once daily, may increase dose to 15 mg once daily. Max dose: 15 mg daily. RA 15 mg daily. Reduce dose to 7.5 mg daily if w/ good response. Acute exacerbation of OA 7.5 mg daily, increased if necessary to max of 15 mg daily. Ankylosing spondylitis 15 mg daily. Dialysis patient w/ severe renal failure 7.5 mg daily.
Administration
Should be taken with food: Swallow w/ a glass of water.
Contraindications
Hypersensitivity. History of urticaria, angioedema, bronchospasm, severe rhinitis or shock precipitated by aspirin or other NSAIDs; aspirin sensitivity, asthma, nasal polyps. Treatment of peri-op pain in CABG surgery. Not to be used in patient w/ renal failure (CrCl ≤20 mL/min).
Special Precautions
Discontinue immediately at 1st appearance of skin rash, fever, eczema, blister or other signs of hypersensitivity. Not to be used in patient w/ hypersensitivity or history of urticaria, angioedema, bronchospasm, severe rhinitis or shock precipitated by aspirin or NSAIDs; dengue fever patient. Risk of GI ulceration & bleeding; serious CV thrombotic events, MI & stroke. May cause cardiac failure or HTN. Monitor patients w/ impaired renal, hepatic or cardiac function including frail or debilitated patients. Patient w/ decreased renal blood flow & vol; who may be adversely affected by platelet function alteration. Concomitant use w/ NSAIDs may enhance renal dysfunction. Patient w/ end-stage renal failure. Not to be used during 3rd trimester of pregnancy. Avoid breastfeeding. Elderly.
Adverse Reactions
Edema; headache, dizziness; rash; diarrhea, dyspepsia, nausea, flatulence, abdominal pain; URTI, pharyngitis; flu-like symptoms.
Drug Interactions
Antagonized antihypertensive effect w/ ACE inhibitors & AIIA. Increased risk of developing renal failure secondary to decreased renal blood flow w/ diuretics; serious GI events w/ aspirin; GI bleeding w/ warfarin. Reduced natriuretic effects of furosemide & thiazides. Increased plasma lithium conc. Enhanced toxicity of MTX.
