Indications/Uses
Treatment of thromboembolic disorders.
Dosage/Direction for Use
Initial dose: 2-5 mg/day. Patient w/ certain genetic variation in CYP2C9 & VKORC1, elderly & debilitated Lower initiation dose. Maintenance dose: 2-10 mg/day. Patient w/ a potential to exhibit greater than expected PT/INR, elderly &/or debilitated Lower maintenance dose.
Administration
May be taken with or without food.
Contraindications
Hemorrhagic tendencies or blood dyscrasias. Recent contemplated surgery of: CNS, eye, traumatic surgery in large open surfaces. Bleeding tendencies associated w/ active ulceration or overt bleeding of GUT, GI or resp tract, cerebrovascular hemorrhage, cerebral aneurysms, dissecting aorta, pericardial & pericarditis effusions, bacterial endocarditis. Threatened abortion: Eclampsia & preeclampsia. Unsupervised patients w/ senility: Alcoholism, psychosis/lack of patient cooperation. Spinal puncture: Other diagnostic or therapeutic procedures w/ potential for uncontrollable bleeding. Major regional, lumbar block anesth, malignant HTN. Pregnancy.
Special Precautions
Serious risk of hemorrhage, hemorrhagic blood disorders, peptic ulcer, severe wounds (eg, surgical wounds), cerebrovascular disorders, bacterial endocarditis.
Adverse Reactions
Drug Interactions
Drugs diminishing the effect of warfarin: Acetamenaphthone, alcohol (chronic ingestion w/o liver impairment), aminoglutethimide, barbiturates, carbamazepine, dichloralphenazone, ethchlorvynol, glutethimide, griseofulvin, nafcillin, phytomenadione, rifampicin. Drugs enhancing the effect of warfarin: Alcohol (acute or chronic ingestion), allopurinol, amiodarone, aspirin, cefamandole, chloramphenicol, cimetidine, clofibrate, chloral hydrate, co-trimoxazole, danazol, dextropropoxyphene, dextrothyroxine, dipyridamole, disulfiram, erythromycin, ethacrynic acid, ethylestrenol, fluconazole, glucagon, itraconazole, ketoconazole, metronidazole, miconazole, norethandrolone, NSAIDs, oxymetholone, quinidine, stanozolol, sulfinpyrazone, tamoxifen, thyroid agents, tienilic acid, triclofos Na.
