Initially 25 mg bid (every 12 hr) on day 1, 50 mg bid (every 12 hr) on day 2, 100 mg bid (every 12 hr) on day 3 & 150 mg bid (every 12 hr) on day 4. Then, adjust to a usual range of 300-450 mg daily in 2 divided doses (every 12 hr).
Contraindicated in patients w/ pre-existing CNS depression or coma, bone marrow suppression, pheochromocytoma or prolactin-dependent tumor. Caution or do not use at all in patients w/ impaired liver, kidney, CV, cerebrovascular & resp function, angle-closure glaucoma, history of jaundice, parkinsonism, DM, hypothyroidism, myasthenia gravis, paralytic ileus, prostatic hyperplasia or urinary retention. Care is required in patients w/ epilepsy or history of seizures. Should be used during pregnancy only if potential benefit justifies potential risk to the fetus. Discontinue nursing while on treatment or discontinue the drug if breastfeeding.
