Clindamycin HCl
Indications/Uses
Infections caused by susceptible anaerobic bacteria, susceptible strains of gm +ve aerobic bacteria (eg, streptococci, staphylococci & pneumococci), & susceptible strains of Chlamydia trachomatis: Upper resp infections including tonsilitis, pharyngitis, sinusitis, otitis media & scarlet fever; lower resp infections including bronchitis, pneumonia, emphysema & lung abscess; skin & soft tissue infections including acne, furuncles, cellulitis, impetigo, abscesses & wound infections, & specific skin & soft tissue infections eg, erysipelas & paronychia; bone & joint infections including osteomyelitis & septic arthritis; gynecological infections including endometritis, cellulitis, vag cuff infection & tubo-ovarian abscess, salpingitis & pelvic inflammatory disease when given in conjunction w/ an antibiotic of appropriate gm -ve aerobic spectrum, & cervicitis due to Chlamydia trachomatis; intra-abdominal infections including peritonitis & abdominal abscesses when given in conjunction w/ an antibiotic of appropriate gm -ve aerobic spectrum; septicemia & endocarditis; dental infections eg, periodontal abscess & periodontitis; toxoplasmic encephalitis & Pneumocystis jirovecii pneumonia in patients w/ AIDS.
Dosage/Direction for Use
Adult 600-1,800 mg daily divided in 2, 3 or 4 equal doses. Childn >1 mth who are able to swallow cap 8-25 mg/kg daily in 3 or 4 equal doses. β-hemolytic streptococcal infection Adult & childn dose. Continue treatment for at least 10 days. Chlamydia trachomatis cervicitis 450-600 mg qid for 10-14 days. Toxoplasmic encephalitis Patient w/ AIDS 600-1,200 mg every 6 hr for 2 wk followed by 300-600 mg every 6 hr. Total duration: 8-10 wk. Pneumocystis carinii pneumonia Patient w/ AIDS 300-450 mg every 6 hr for 21 days. Acute streptococcal tonsilitis/pharyngitis 300 mg bid for 10 days.
Administration
May be taken with or without food: Take w/ full glass of water.
Contraindications
Previous hypersensitivity to clindamycin or lincomycin.
Special Precautions
Discontinue use if a hypersensitivity or severe skin reaction [eg, DRESS, SJS, TEN, & acute generalized exanthematous pustulosis (AGEP)] occurs. Pseudomembranous colitis. Clostridium difficile-associated diarrhea. Perform liver & kidney function tests in prolonged therapy. Effect on ability to drive or operate machinery has not been systematically evaluated. Only use in pregnancy if clearly needed. Lactation. Not suitable for childn who are unable to swallow cap whole.
Adverse Reactions
Pseudomembranous colitis, Clostridium difficile colitis, vag infection; agranulocytosis, neutropenia, thrombocytopenia, leukopenia, eosinophilia; anaphylactic shock & reaction, anaphylactoid reaction, hypersensitivity; dysgeusia; diarrhea, abdominal pain, esophageal ulcer, esophagitis, vomiting, nausea; jaundice; TEN, SJS, DRESS, AGEP, angioedema, erythema multiforme, pruritus, morbilliform rash; abnormal LFT.
Drug Interactions
May enhance action of other neuromuscular blocking agents. Clearance may be reduced w/ CYP3A4 & CYP3A5 inhibitors & be increased w/ CYP3A4 & CYP3A5 inducers. Monitor for loss of effectiveness in presence of CYP3A4 inducers eg, rifampicin.
