Glyceryl trinitrate
Indications/Uses
Anal fissure & symptom relief associated w/ anal fissure. Pain relief & to aid wound healing as part of post-op management following haemorrhoidectomy.
Dosage/Direction for Use
Adult Insert 1-1.5 cm strip of oint tds into the anal canal.
Contraindications
Hypersensitivity. Severe anaemia, glaucoma, hypotension, increased ICP, idiosyncrasy to glyceryl trinitrate, migraine or recurrent headache, aortic or mitral stenosis, hypertrophic obstructive cardiomyopathy, constrictive pericarditis or pericardial tamponade, postural hypotension & uncorrected hypovolaemia. Not to be co-administered w/ sildenafil citrate, tadalafil or vardenafil. Pregnancy & lactation.
Special Precautions
Can interfere w/ measurement of catecholamines & vanilmandelic acid in urine; can increase excretion of these substances. Discontinue treatment if anal bleeding increases. Advise patients to slowly change position when changing from lying/sitting to upright; to minimize postural hypotension especially in those w/ low blood vol & under diuretic treatment. Paradoxical bradycardia & increased angina pectoris may accompany GTN-induced hypotension. Carefully monitor patients w/ acute MI or CHF to avoid hypotension & tachycardia. May impair ability to drive or operate machinery especially at the start of treatment due to faintness &/or dizziness particularly in the elderly. Childn.
Adverse Reactions
Drug Interactions
Vascular effects may be enhanced by alcohol. May potentiate BP lowering effects w/ other vasodilators, Ca channel blockers, ACE inhibitors & β-blockers. Acetyl cysteine may potentiate vasodilatory effects of glyceryl trinitrate. Decreased heparin efficacy; closely monitor blood coagulation parameters. May reduce thrombolytic activity of alteplase. May increase bioavailability of dihydroergotamine which can lead to coronary vasoconstriction. Possible diminished therapeutic response w/ ingestion of ASA & NSAIDs. Potential tachyphylaxis; concurrent treatment w/ anti-anginal products containing nitrates may interfere w/ nitrate free period dosage arrangements for the anti-anginal drug.
