Cefotaxime
Indications/Uses
Infections of the lower resp tract including pneumonia; GUT including uncomplicated gonorrhea; gynecologic including pelvic inflammatory disease, endometritis & pelvic cellulitis; intra-abdominal including peritonitis; CNS eg, meningitis & ventriculitis; bacteremia, septicemia; skin & skin structure; bone &/or joint.
Dosage/Direction for Use
Dose may be given by deep IM inj or IV by slow inj over 3-5 min or by infusion over 20-60 min. Adult Usual dose: 2-6 g daily in 2-4 divided doses. Severe infections Up to 12 g IV daily in 6 divided doses. Pseudomonal infections >6 g daily. Gonorrhoea 500 mg or 1 g as a single dose. Gonococcal urethritis/cervicitis in male & female, & rectal gonorrhea in female 500 mg IM once daily. Rectal gonorrhea in male 1 g IM once daily. Uncomplicated infections 1 g IM/IV bid. Moderate to severe infections 1-2 g IM/IV every 8 hr. Infections commonly needing antibiotics in higher dosage (eg, septicemia) 2 g IV every 6-8 hr. Life-threatening infections 2 g IV every 4 hr. Add appropriate anti-chlamydial coverage if C. trachomatis is suspected. Surgical infection prophylaxis 1 g 30-90 min before surgery. Caesarean section 1 g IV as soon as the umbilical cord is clamped & 2 further doses IM/IV 6-12 hr later. Childn 100-150 mg/kg daily in 2-4 divided doses. Severe infections 200 mg/kg daily. Childn & infant (1 mth-12 yr) ≥50 kg Usual adult dose, not exceeding 12 g, <50 kg 50-180 mg/kg IM/IV, divided into 4-6 equal doses.Neonate 50 mg/kg daily in 2-4 divided doses. Severe infections 150-200 mg daily, 1-4 wk 50 mg/kg IV every 8 hr, 0-1 wk 50 mg/kg IV every 12 hr. Renal impairment Initially 1 g, halving the dose while maintaining usual frequency of dose has been suggested.
Contraindications
Hypersensitivity to cephalosporins.
Adverse Reactions
Hypersensitivity reactions, including skin rashes, urticaria, eosinophilia, fever, serum sickness-like reactions, anaphylaxis. Arrhythmias w/ rapid bolus administration.
Drug Interactions
Reduced renal clearance w/ probenecid. Reduced total body clearance w/ ureidopenicillins azlocillin or mezlocillin.
