Congestive heart failure of any severity.
Hemostatic disorders or active pathologic bleeding, such as bleeding peptic ulcer or intracranial bleeding.
History of ventricular arrhythmias.
QT interval prolongation.
Severe renal impairment or moderate to severe hepatic impairment.
