Childn >11 yr 25-50 mg 30 min before bedtime & treatment including gradual w/drawal period should not be >3 mth, 6-10 yr 10-20 mg 30 min before bedtime & treatment including gradual w/drawal period should not be >3 mth.
Patients w/ urinary retention, prostatic hyperplasia, or chronic constipation; untreated angle-closure glaucoma & phaeochromocytoma; history of epilepsy; CV disease; hyperthyroidism. Avoid in patients w/ heart block, cardiac arrhythmias, or in immediate recovery period after MI. Suicidal thoughts & behaviour may develop during early treatment for other disorders. May precipitate mania when used for depressive component of bipolar disorders & may aggravate psychotic symptoms when used as depressive component of schizophrenia. Pregnancy & lactation. Childn & adolescents w/ major depressive disorder & other psychiatric disorders.
Not to be given to patients on MAOIs or for at least 2 wk after their w/drawal. At least 1-2 wk should elapse between TCA w/drawal & starting any drug liable to provoke a serious reaction (eg, phenelzine).
