Indications/Uses
Maintenance treatment of patients w/ COPD (including chronic bronchitis & emphysema); associated dyspnoea & for prevention of exacerbations. Spiriva Respimat For the improvement of COPD compromised quality of life & reduction of exacerbations. Add-on maintenance treatment for the improvement of asthma symptoms, quality of life, & reduction of exacerbations in patients ≥6 yr w/ asthma who remain symptomatic on at least inhaled corticosteroids.
Dosage/Direction for Use
Spiriva 1 cap inhaled once daily via Handihaler (1 puff once daily at the same time of day). Spiriva Respimat 2 puffs once daily at the same time of day. Childn 6-17 yr Asthma 2 puffs once daily at the same time of day.
Contraindications
Hypersensitivity to tiotropium Br, atropine or its derivatives (eg, ipratropium or oxitropium).
Special Precautions
Immediate hypersensitivity reactions may occur. Not for initial treatment of acute episodes of bronchospasm or for relief of acute symptoms. Patients w/ narrow-angle glaucoma, prostatic hyperplasia or bladder-neck obstruction. May cause inhalation-induced bronchospasm. Not to be used more frequently than once daily. Occurrence of dizziness or blurred vision may influence the ability to drive & use machinery. Patients w/ moderate to severe renal impairment (CrCl ≤50 mL/min). Preferable to avoid use during pregnancy. Not to be used in pregnant or nursing women unless expected benefit outweighs any possible risk to unborn child or infant. Spiriva Do not swallow cap. Avoid the powd from entering the eyes. Use cap only w/ the HandiHaler device. Contains lactose monohydrate. Not to be used in infants & childn. Spiriva Respimat Not to be used as 1st-line treatment for asthma. Avoid the soln or mist from entering the eyes. Use cartridges only w/ the Respimat reusable inhaler. Contains benzalkonium Cl which may cause wheezing & breathing difficulties especially in patients w/ asthma. Efficacy & safety has not been established in paed patients <1 yr w/ asthma.
Adverse Reactions
Dehydration; dizziness, insomnia; blurred vision, glaucoma, increased IOP; atrial fibrillation, supraventricular tachycardia, tachycardia, palpitations; cough, dysphonia, pharyngitis, bronchospasm, epistaxis, laryngitis, sinusitis; usually mild dry mouth, constipation, GERD, oropharyngeal candidiasis, intestinal obstruction including paralytic ileus, dysphagia, gingivitis, glossitis, stomatitis; rash, angioedema/angioneurotic oedema, hypersensitivity (including immediate reactions), pruritus, urticaria, dry skin, skin infection & ulcer; joint swelling; urinary retention (usually in men w/ predisposing factors), dysuria, UTI.
Drug Interactions
Not recommended w/ chronic co-administration of other anticholinergic drugs.
