Piperacillin Na 4 g, tazobactam Na 500 mg.
Indications/Uses
For documented multi-drug resistant gm -ve infections due to organisms proven or suspected to be susceptible to piperacillin/tazobactam except CNS infections; for polymicrobial infections (eg, mixed aerobic & anaerobic infections).
Dosage/Direction for Use
IV infusion over 30 min. Adult 3.375 g every 6 hr totalling 13.5 g. Initial presumptive treatment of patients w/ nosocomial pneumonia 16 g piperacillin/2 g tazobactam. Childn >40 kg 4.5 g every 6 hr (16 g piperacillin/2 g tazobactam). CAP, intra-abdominal infections (appendicitis &/or peritonitis, uncomplicated & complicated skin & skin structure infections, pelvic inflammatory disease 3.375 g every 6 hr (16.0 gm Piperacillin/2.0 gm Tazobactam per day) + aminoglycoside. ≥9 mth weighing up to 40 kg CAP, intra-abdominal infections (appendicitis &/or peritonitis, uncomplicated & complicated skin & skin structure infections, pelvic inflammatory disease 100 mg piperacillin + 12.5 mg tazobactam/kg every 8 hr (3.375 g every 6 hr). 2-9 mth CAP, intra-abdominal infections (appendicitis &/or peritonitis, uncomplicated & complicated skin & skin structure infections, pelvic inflammatory disease 80 mg piperacillin/10 mg tazobactam/kg every 8 hr. Usual duration of therapy: 7-10 days; nosocomial pneumonia: 7-14 days.
Contraindications
Hypersensitivity to penicillins, cephalosporins, β-lactamase antibiotics, or β-lactamase inhibitors.
Special Precautions
Hypersensitivity. Cystic fibrosis, bowel inflammation, bleeding problems, CHF or kidney problems; patient on dialysis; history of severe diarrhea or bowel problems due to antibiotics, & if on a salt-restricted diet or have low blood K levels. Concomitant use w/ herbal prep or dietary supplement. Pregnancy & lactation.
Adverse Reactions
Agitation; constipation; diarrhea; dizziness; headache; indigestion; nausea; pain; swelling or redness at inj site; sleeplessness; vomiting. Severe allergic reactions (rash; hives; itching; difficulty of breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); bloody stools; chest pain; decreased urination; depression; fainting; fast, slow or irregular heartbeat; fever; chills or sore throat; hallucination; inflammation at inj site; prolonged muscle relaxation; red, swollen or blistered skin; seizure; severe diarrhea, vomiting, or stomach pain; shortness of breath; swelling of the hands, ankles, or feet; tremor; unusual tiredness or weakness; vag irritation or discharge; vein inflammation or tenderness; yellowing of the eyes or skin.
Drug Interactions
Decreased effectiveness of aminoglycosides (eg, tobramycin) or OCs; warfarin. Increased blood K levels w/ chemotherapy or diuretics (eg, furosemide, hydrochlorothiazide). Concomitant use increases side effects of heparin, MTX, or nondepolarizing muscle relaxants (eg, vecuronium). Increased risk of side effects w/ probenecid. Decreased effectiveness w/ tetracyclines (eg, doxycycline).
