Ursodeoxycholic acid
Indications/Uses
Primary biliary cirrhosis & dissolution of small to medium sized radiolucent, cholesterol-rich gallstones in patients w/ a functioning gallbladder.
Dosage/Direction for Use
Adult & elderly 10-15 mg/kg daily in 2-4 divided doses.
Contraindications
Calcified & pigment/radiopaque gallstones; nonfunctioning gallbladders; chronic liver & PUD; inflammatory disease of colon & small intestine. Pregnancy.
Special Precautions
Do not use if the gallbladder cannot be visualized on x-ray images or in cases of calcified gallstones, impaired contractility of the gallbladder or frequent episodes of biliary colic. Monitor liver function parameters [eg, AST (SGOT), ALT (SGPT) & γ-GT] every 4 wk during the 1st 3 mth of treatment, & thereafter every 3 mth. Visualize gallbladder (oral cholecystography) w/ overview & occlusion views in standing & supine positions (ultrasound control) 6-10 mth after beginning treatment. Reduce dose if diarrhea occurs & discontinue use in cases of persistent diarrhea. Treat women of childbearing potential only if they are on reliable contraception. Not to be taken during lactation.
Adverse Reactions
Drug Interactions
Effectiveness may be reduced w/ cholestyramine, charcoal & antacids. Absorption may be reduced w/ Al-based antacids. Effectiveness may be counteracted by oestrogens & clofibrate. Possible increased serum conc of ciclosporin. Decreased effectiveness of dapsone. Possible decreased serum ciprofloxacin & nitrendipine.
