Per Prazitam-2.25 Piperacillin Na 2 g, tazobactam Na 250 mg. Per Prazitam-4.5 Piperacillin Na 4 g, tazobactam Na 500 mg
Indications/Uses
For documented multidrug resistant gm -ve infections due to organisms proven or suspected to be susceptible to piperacillin/tazobactam except CNS infections & for polymicrobial infections (eg, mixed aerobic & anaerobic infections in which other agents have insufficient activity or are contraindicated due to toxic potential).
Dosage/Direction for Use
IV infusion over 30 min. IM/IV Adult 3.375 g daily every 6 hr. Total daily dose: 13.5 g. Nosocomial pneumonia Initially 16 g/2 g daily for 7-14 days. Continue aminoglycoside to patients from whom P. aeruginosa is isolated. Community-acquired pneumonia, intra-abdominal, uncomplicated & complicated skin & skin structure infections, pelvic inflammatory disease Childn ≥40 kg 3.375 g every 6 hr + aminoglycoside, ≥9 mth up to 40 kg 100 mg/12.5 mg/kg every 8 hr, 2-9 mth 80 mg/10 mg/kg every 8 hr. Duration of treatment: 7-10 days. Hemodialysis All indications Max: 2.25 g every 12 hr. Nosocomial pneumonia 2.25 g every 8 hr. Additional dose of 0.75 g Prazitam should be administered following each dialysis period on hemodialysis days; no additional dosage for CAPD patients.
Contraindications
Hypersensitivity to piperacillin, tazobactam, penicillins, cephalosporins, β-lactamase antibiotics, tetracyclines, or β-lactamase inhibitors.
Special Precautions
Allergies to medicines, food or other substances. Cystic fibrosis, bowel inflammation, bleeding problems, CHF or kidney problems. Patients on dialysis or w/ history of severe diarrhea or bowel problems due to antibiotics. Patient on salt-restricted diet or w/ low blood K levels. Concomitant use of other medicines, herbal prep or dietary supplement. Pregnancy & lactation.
Adverse Reactions
Agitation; constipation; diarrhea; dizziness; headache; indigestion; nausea; pain; swelling or redness at the inj site; sleeplessness; vomiting. Severe allergic reactions (rash; hives; itching; difficulty of breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); bloody stools; chest pain; decreased urination; depression; fainting; fast, slow, or irregular heartbeat; fever, chills, or sore throat; hallucination; inflammation at the inj site; prolonged muscle relaxation; red, swollen, or blistered skin; seizure; severe diarrhea, vomiting, or stomach pain; shortness of breath; swelling of the hands, ankles or feet; tremor; unusual tiredness or weakness: vag irritation or discharge; vein inflammation or tenderness; yellowing of the eyes or skin.
Drug Interactions
Decreased effectiveness of aminoglycosides (eg, tobramycin) or OCs (birth control pills). Decreased effectiveness or decreased/increased risk of side effects of anticoagulants (eg, warfarin). Increased risk of side effects eg, low blood K levels of chemotherapy or diuretics (eg, furosemide, hydrochlorothiazide). Increased risk of side effects of heparin, MTX or nondepolarizing muscle relaxants (eg, vecuronium). Increased risk of side effects w/ probenecid. Decreased effectiveness w/ tetracyclines (eg, doxycycline).
