Indications/Uses
Acute chronic bronchitis, bronchiectasis (w/ infection), pulmonary abscesses, bacterial pneumonia, chest infection post-op, paranasal sinusitis, tonsillitis, acute chronic pyelonephritis, cystitis, pharyngitis, bacterial urine w/o symptoms, cellulitis, erysipelas peritonitis, infection of lanceolate, septic arthritis, myelitis sepsis, gonorrhea, meningitis, prevention from infection after operations of abdominal region, pelvis, orthopedic treatment, heart, lung, esophagus, blood vessel operation. Used in intrauterine infection, bartholinitis.
Dosage/Direction for Use
Adult 750 mg tid IV or IM. Severe case Dose may be raised to 1.5 g tid IV. If necessary, frequency may be raised to 4 times & total daily dose is 3-6 g. Childn & infant 30-100 mg/kg daily 3-4 times in divided dose by IM or IV. Usually, reasonable for 60 mg/kg daily. Neonate 30-100 mg/kg daily 2-3 times in divided doses by IM or IV. Administer carefully during 1st wk of birth. Gonorrhea 1.5 g once IM or could equally administer 750 mg in both side of the breech. Mild renal failure case (10-20 mL/min CrCl) 750 mg bid. Severe renal failure case (<10 mL/min CrCl) 750 mg daily. Hemodialysis case 750 mg more after hemodialysis. Performed continuously artery & venous dialysis or high fast hematofiltration 750 mg bid. Protection after abdominal, pelvis, orthopedic treatment operation 1.5 g IV in usually inducing anesth & can add to 750 mg IM each time after 8 hr & 16 hr. Heart, lung, esophagus, or blood vessel operation 750 mg daily for 24-48 hr 3 times by IM, after administering 1.5 g by IV including anesth. Total joint replacement Before adding liqd monomer, may use to mix cefuroxime powd 1.5 g w/ each pack of methyl methacrylate cement polymer. Adult Meningitis 3 g tid by IV. Meningitis in childn & infant 200-240 mg/kg daily 3-4 times in divided doses by IV reduced to 10 mg/kg if symptoms improved in 3 days. Meningitis in neonate Initially 100 mg/kg daily by IV, reduced to 50 mg/kg daily if symptoms improved.
Contraindications
Patients w/ history of shock to cefuroxime Na or axetil.
Special Precautions
Patient history should be taken & skin reaction test should be performed to predict adverse reaction eg, shock. Perform lab tests (hepatic/renal function, blood) at regular intervals. Permitted to administer w/ aminoglycosides & metronidazole, but do not mix in the same syringe.
Adverse Reactions
Shock; SJS, erythema multiforme syndrome & Lyell syndrome (toxic epidermonecrosis); pruritus, exanthema (erythema, wheal) fever, urticaria, lymphadenopathy, arthralgia, anaphylactic reaction; renal insufficiency; reduction of Hb conc, anemia, granulocytopenia, thrombocytopenia, neutropenia, eosinophilia, hemolytic anemia; elevation of ALT, AST, AL-P, bilirubin, LDH, y-GPT; severe enterocolitis w/ hemafecia eg, pseudomembranous enterocolitis, abdominal pain, diarrhea, nausea, vomiting, anorexia; interstitial pneumonia, PIE syndrome. accompanied w/ fever, cough, abnormal chest x-ray, dyspnea; candidal intertrigo; symptoms of vit K deficiency (hypoprothrombinemia, hemorrhage frequency) & vit B deficiency (glossitis, stomatitis, anorexia, neuritis); numbness.
Drug Interactions
Cautious treatment is required as renal insufficiency maybe worsened w/ combination of similar compd (other aminoglycosides) & diuretics eg, furosemide.
