IV infusionPatient >33 kg to ≤50 kg 15 mg/kg (1.5 mL/kg). Max: 75 mL (60 mg/kg not exceeding 3 g) daily. >10 kg to ≤33 kg 15 mg/kg (1.5 mL/kg). Max: 49.5 mL (60 mg/kg not exceeding 2 g) daily. ≤10 kg 7.5 mg/kg (0.75 mL/kg). Max: 7.5 mL (30 mg/kg) daily. Patient >50 kg & no additional risk factors of hepatotoxicity 1 g (100 mL/dose). Max: 4 g/day. >50 kg w/ additional risk factors of hepatotoxicity 1 g (100 mL/dose). Max: 3 g/day. Min interval between each administration must be at least 4 hr & at least 6 hr in patient w/ severe renal insufficiency. Adult w/ hepatocellular insufficiency, chronic alcoholism & malnutrition, dehydration Max: 3 g/day.
Serious liver damage in high doses. May cause severe skin reactions. Hepatocellular & severe renal (CrCl ≤30 mL/min) insufficiency; chronic alcoholism & malnutrition; dehydration. Anaphylactic symptoms & life-threatening or less severe asthmatic episodes due to Na sulfite content. Pregnancy & lactation.
May cause 2-fold reduction in clearance w/ probenecid. May prolong elimination t½ w/ salicylamide. Caution w/ concomitant use w/ enzyme-inducing substances. May lead to slight variations of INR values w/ oral anticoagulants.
