Contents
Fe sulfate 525 mg (equiv to elemental Fe 105 mg), ascorbic acid 500 mg, niacinamide 30 mg, Ca pantothenate 10 mg, thiamine mononitrate 6 mg, riboflavin 6 mg, pyridoxine HCl 5 mg, cyanocobalamin 25 mcg
Indications/Uses
Prevention & treatment of Fe deficiency anemia, or for treatment of Fe deficiency especially when there is concomitant need for vit C or the B vit.
Dosage/Direction for Use
Adult including pregnant women 1 tab daily.
Administration
Should be taken on an empty stomach: Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity. Patients w/ thalassemia, sideroblastic anemia, hemochromatosis & hemosiderosis. Repeated blood transfusions or parenteral Fe therapy. Intestinal diverticula or any intestinal obstruction. Niacinamide: Patients w/ liver disease or active peptic ulcer; arterial hemorrhaging or severe hypotension. Childn.
Special Precautions
Fe therapy could induce relapse of erythropoietic protoporphyria. Nature of existing anemia should be established. Childn <6 yr. Ascorbic acid: Patients w/ chronic renal failure & receiving ASA. Pregnancy & lactation.
Adverse Reactions
Nausea, vomiting, abdominal pain or discomfort, diarrhea or constipation. Allergic reactions including rash, pruritus & anaphylaxis. GI effects eg, heartburn, eructation, abdominal pain & cramps, diarrhea, vomiting, nausea & anorexia. Hepatic dysfunction w/ abnormal LFTs including hyperbilirubinemia. Deterioration of acneiform vulgaris or eruption of acneiform exanthema. Fe: Black discoloration of the stool. Increased capillary permeability, reduced plasma vol, increased cardiac output & sudden CV collapse. Ascorbic acid: Stone formation, crystalluria & oxalosis. Diarrhea & other GI disturbances; formation of renal Ca oxalate calculi. Niacinamide: Strong vasodilator effects (eg, flushing, dizziness or faintness). Riboflavin: Bright yellow urine discoloration. Pyridoxine: Peripheral sensory neuropathies.
Drug Interactions
Fe: Inhibits absorption of tetracyclines. May delay response to Fe therapy w/ chloramphenicol. May decrease absorption w/ antacids & cholestyramine. May interfere w/ the oral absorption of some quinolones (eg, ciprofloxacin, norfloxacin, ofloxacin). Decreased cupruretic effect of penicillamines. Inhibited absorption w/ eggs, milk, phytate-containing substances eg, bran or tannins in tea. Hinder detection of occult blood in feces. False +ve for guaiac test. Ascorbic acid: Decreased prothrombin time w/ anticoagulants. Interfere w/ tests based on oxidation-reduction reactions. May produce false results in glucosuria determinations in the urine. Niacinamide: Toxic delirium & lactic acidosis w/ alcohol. Increased flushing & dizziness w/ nicotine. May potentiate the hypotensive effect of ganglionic blocking drugs. May enhance the effects of neuromuscular blocking agents. May cause false elevations in fluorometric tests of urinary catecholamines. False +ve results when cupric sulfate soln (eg, Benedict's reagent) is used for urinary glucose determination. May change dosage requirements of anti-diabetic agents. Thiamine: False +ve results in the phosphotungstate method for determination of uric acid. False +ve results for urobilinogen in the spot test using Ehrlich's reagent. Riboflavin: May delay absorption time but increase the absorption conc w/ propantheline Br. May interfere w/ urinalysis based on spectrometry or color reactions. False elevations in fluorometric determinations of catecholamines or urobilinogen. Pyridoxine HCl: Decreased serum conc of phenobarb or phenytoin. False +ve results for urobilinogen in the spot test using Ehrlich's reagent. Possible interaction w/ levodopa. Cyanocobalamin: May diminish response w/ chloramphenicol. Diminished absorption w/ aminoglycosides, colchicine, ER K prep, aminosalicylic acid & its salts, anticonvulsants, cobalt irradiation of the small bowel or chronic excessive alcohol intake. Invalidate diagnostic microbiologic blood assays w/ MTX, pyrimethamine & most anti-infectives. False +ve test results for IF Abs.
