Indications/Uses
Treatment & prevention of folate deficiency state. Protection against neural tube defects in offspring of women of child-bearing potential & pregnant women. Megaloblastic anemia.
Dosage/Direction for Use
Adult Folate deficient megaloblastic anemia & drug-induced folate deficiency 5 mg daily for 4 mth; up to 15 mg daily for malabsorption states. Prophylaxis in chronic hemolytic states or in renal dialysis 5 mg every 1-7 days depending on underlying disease. Prevention of neural tube defects in women known to be at risk 5 mg daily started before conception & continued throughout 1st trimester. Established folate deficiency in pregnancy 5 mg daily continued to term. Childn 12-18 yr Folate deficiency anemia 1 tab (5 mg) daily for 4 mth, maintenance dose 5 mg. Haemolytic anemia & metabolic disorder 1-2 tab (5-10 mg) daily. Prevention of folate deficiency in patient w/ kidney dialysis 1-2 tab (5-10 mg) daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Folate deficiency due to dihydrofolate reductase inhibitors, pernicious, aplastic or normocytic anemia.
Special Precautions
Not to be given alone or w/ inadequate vit B12 for the treatment of pernicious anemia or undiagnosed megaloblastic anemia & in patients w/ folate-dependent tumors. Large & continuous folic acid dosage may lower vit B12 blood conc. Availability of folates may be inhibited by gastric acidity & presence of ascorbate in GIT. Concomitant use w/ metformin or phenytoin.
Adverse Reactions
Drug Interactions
Reduced intestinal absorption of folic acid w/ metformin. Folate deficiency states may be produced by drugs eg, antiepileptics, OC, anti-TB drugs, alcohol, & folic acid antagonists eg, MTX, pyrimethamine, triamterene, trimethoprim & sulfonamides. Reports of decreased serum phenytoin conc w/ folate supplementation. Reduction or delay in folic acid absorption w/ cholestyramine.
