Paracetamol 500 mg, thiamine mononitrate (vit B₁) 50 mg, pyridoxine HCl (vit B₆) 100 mg, cyanocobalamin (vit B₁₂) 100 mcg

Indications/Uses

Acute & painful neuritis & neuralgia, especially cervical & shoulder arm syndrome, lumbalgia, intercostal neuralgia, ischialgia, trigeminal neuralgia, herpes zoster, post-op pain.

Dosage/Direction for Use

Therapeutic 1-2 tab tid. Maintenance 1-2 tab daily.

Overdosage

Administration

Contraindications

Acute intermittent porphyria, decompensated cardiac insufficiency, AV block, bradycardia.

Special Precautions

Pregnancy & lactation. Vit B₁₂: Administration of >10 mcg may produce hematological response in patients w/ folate deficiency. Paracetamol: Liver & hepatic impairment; alcohol dependence.

Adverse Reactions

Paracetamol: Hematological reactions including thrombocytopenia, leucopenia, pancytopenia, neutropenia, & agranulocytosis; skin rashes & other hypersensitivity reactions (occasionally). Vit B₆: Development of severe peripheral neuropathy (long term use & large doses).

Drug Interactions

Paracetamol: Increased risk of toxicity w/ hepatotoxic drugs or drugs that induce liver microsomal enzymes. Increased absorption w/ metoclopramide. Altered plasma conc & excretion may be affected w/ probenecid. Reduced absorption w/ colestyramine. Vit B₆: Reduces effect of l-dopa. Reduces altretamine activity. Decrease serum conc of phenobarb & phenytoin. May increase requirements for pyridoxine w/ hydralazine, INH, penicillamine & OCs. Vit B₁₂: Reduced absorption from GI tract w/ neomycin, aminosalicylic acid, H₂-antagonists, omeprazole & colchicine. Decreased serum conc w/ OCs.
