Valproic acid
Indications/Uses
Primary generalized seizures. Has notable benefit in absence & myoclonic seizures. Partial seizures. Acute manic phase of bipolar disorder. Prophylaxis of migraine.
Dosage/Direction for Use
Mania Initially 25 mg/kg daily, increased rapidly to achieve therapeutic response or desired plasma level. Max: 60 mg/kg daily. Complex partial seizure Initially 10-15 mg/kg daily, increased at 1 wk intervals by 5-10 mg/kg daily to achieve optimal clinical response. Max: 60 mg/kg daily. Absence seizure Initially 15 mg/kg daily, increased at 1-wk intervals by 5-10 mg/kg daily until seizure control or limiting side effects. Max: 60 mg/kg daily. Migraine Initially 500 mg daily for 1 wk, increased thereafter to 1,000 mg daily.
Administration
May be taken with or without food: Swallow whole, do no crush/chew.
Contraindications
Hypersensitivity. Patients w/ known urea cycle disorders. Not to be administered to patients w/ hepatic disease or significant hepatic dysfunction.
Special Precautions
Immediately discontinue use in presence of suspected or apparent significant hepatic dysfunction. Risk of developing hepatic failure &/or clotting abnormalities. Patients w/ prior history of hepatic disease; on multiple anticonvulsants, those w/ congenital metabolic disorders, severe seizure disorders accompanied by mental retardation, & organic brain disease. Do not abruptly discontinue use. Perform LFTs prior to therapy & at frequent intervals thereafter, especially during the 1st 6 mth. Increased risk of congenital malformations w/ maternal use during pregnancy. Use w/ extreme caution & as sole agent in childn <2 yr.
Adverse Reactions
GI disturbances particularly at start of therapy; increased appetite & wt gain; elevation of liver enzyme values (normally transient & dose-related).
Drug Interactions
Raised blood conc & symptoms consistent w/ toxicity w/ erythromycin. Antiepileptic activity may be antagonized w/ antidepressant. Increased bioavailability w/ antacid (Al & Mg hydroxides). Free plasma conc may be increased w/ highly protein-bound drugs.
