Indications/Uses

Moderate to severe Alzheimer's disease.

Dosage/Direction for Use

Adult Dose titration: Initially 5 mg daily, stepwise increased over the 1st 4 wk of treatment as follows: Wk 1 (day 1-7): 5 mg daily for 7 days; wk 2 (day 8-14): 10 mg daily for 7 days; wk 3 (day 15-21): 15 mg daily for 7 days; wk 4 (day 22-28): 20 mg daily for 7 days. Max daily dose: 20 mg. Maintenance dose: 20 mg daily. Elderly >65 yr 20 mg daily. Moderate renal impairment (CrCl 30-49 mL/min) 10 mg daily, may be increased to 20 mg daily if tolerated well after at least 7 days of treatment. Severe renal impairment (CrCl 5-29 mL/min) 10 mg daily.

Overdosage

Administration

May be taken with or without food: Take at the same time each day.

Special Precautions

Patients w/ epilepsy, former history of convulsions or w/ predisposing factors for epilepsy; recent MI, uncompensated CHF (NYHA III-IV), or uncontrolled HTN. Carefully monitor patient for urine pH elevation. Avoid concomitant use of N-methyl-D-aspartate (NMDA)-antagonists eg, amantadine, ketamine or dextromethorphan. Minor or moderate influence on ability to drive & use machines. Not recommended in severe hepatic impairment. Not to be used during pregnancy. Women under treatment should not breastfeed.

Adverse Reactions

Drug hypersensitivity; somnolence; dizziness, balance disorders; HTN; dyspnoea; constipation; elevated LFT; headache.

Drug Interactions

May enhance effects of L-dopa, dopaminergic agonists, & anticholinergics. May reduce effects of barbiturates & neuroleptics. May modify effects of antispasmodic agents, dantrolene or baclofen. Risk of pharmacotoxic psychosis w/ amantadine & potentially ketamine, dextromethorphan & phenytoin. Potential risk of increased plasma levels w/ other active substances that use the same renal cationic transport system eg, cimetidine, ranitidine, procainamide, quinidine, quinine & nicotine. Possible reduced serum level of hydrochlorothiazide. Increased INR w/ warfarin.
