Indications/Uses
Treatment & prophylaxis of anaerobic bacteria & mixed bacterial infections, especially intra-abdominal & pelvic infections, & which include endometriosis (prophylaxis at cesarean section), pelvic inflammatory disease, & surgical infection (prophylaxis). Gonorrhea & UTI.
Dosage/Direction for Use
Given by deep IM inj, slow IV inj over 3-5 min, or intermittent or continuous IV infusion. Adult 1 or 2 g every 8 hr. Severe infection Up to 12 g daily. Childn 20-40 mg/kg every 6-8 hr in childn & older infant, 20-40 mg/kg every 8 hr in neonate 1-4 wk old, & 20-40 mg/kg every 12 hr in neonate up to 1 wk old. Severe infection Up to 200 mg/kg daily. Max: 12 g daily. Specific treatment: Uncomplicated UTI 1 g IM bid. Uncomplicated gonorrhea 2 g IM as single dose w/ oral probenecid 1 g. Surgical infection prophylaxis Adult 2 g IM or IV 30-60 min before procedure, then every 6 hr for up to 24 hr. Infant/childn 30-40 mg/kg at same interval as adult. Neonate 30-40 mg/kg every 8-12 hr. Cesarean section 2 g IV as single dose after umbilical cord clamping, or optional 3-dose regimen w/ further 2 g doses 4 & 8 hr later. Renal impairment In adults, after an initial loading dose of 1-2 g, maintenance doses are: CrCl 30-50 mL/min 1-2 g every 8-12 hr, 10-29 mL/min 1-2 g every 12-24 hr, 5-9 mL/min 0.5-1 g every 12-24 hr, <5 mL/min 0.5-1 g every 24-48 hr, hemodialysis patient Repeat loading dose after each session.
Contraindications
Hypersensitivity to cefoxitin or other cephalosporins.
Special Precautions
Significant changes in anaerobic, facultative, & aerobic flora. Monitor renal & hematological status, especially during prolonged or high-dose therapy. May produce falsely high values in Jaffe method. Lactation.
Adverse Reactions
Skin rashes, urticaria, eosinophilia, fever, reactions resembling serum sickness, anaphylaxis. +ve Coombs test (rare hemolytic anemia), neutropenia, thrombocytopenia (occasional), agranulocytosis (rarely w/ some cephalosporins), bleed complications (related to hypoprothrombinemia &/or platelet dysfunction).
Drug Interactions
Reduced renal clearance w/ probenecid.
