Betamethasone valerate
Indications/Uses
Relief of inflammatory & pruritic manifestations of steroid responsive dermatoses including atopic (infantile atopic dermatitis), nummular (discoid eczema), seborrheic, irritant or allergic dermatitis; prurigo nodularis; psoriasis (excluding widespread plaque psoriasis); lichen simplex chronicus (neurodermatitis) & planus; discoid lupus erythematosus; adjunct to systemic steroid therapy in generalized erythroderma; insect bite reactions & miliaria (prickly heat).
Dosage/Direction for Use
Adult, elderly & childn >1 yr Apply thinly to affected area once daily to bid for up to 4 wk, until improvement occurs.
Contraindications
Untreated cutaneous infections, rosacea, acne vulgaris, pruritus w/o inflammation, perianal & genital pruritus, perioral dermatitis. Dermatoses including dermatitis in infants <1 yr.
Special Precautions
Patients w/ history of local hypersensitivity to corticosteroids. Risk factors that may increase systemic absorption of topical steroids. W/draw gradually if manifestations of hypercortisolism (Cushing's syndrome) & reversible hypothalamic-pituitary-adrenal axis suppression are observed. Institute appropriate antimicrobial therapy in case of infection, & w/draw topical corticosteroid if there is spread of infection. Avoid abrupt w/drawal. Reports of visual disturbance. Risk of bacterial infection w/ occlusion. Reports of rebound relapses, development of tolerances, risk of generalised pustular psoriasis & development of local or systemic toxicity when used in psoriasis. Prolonged application to the face is undesirable. If applied to eyelids, ensure that the prep does not enter the eye. Higher occurrence of local hypersensitivity reactions & increased risk of local infection when used for the treatment of dermatitis in chronic leg ulcers. Contains paraffin; avoid smoking or being near an open flame during application & immediately after use. Patients w/ renal or hepatic impairment. Pregnancy & lactation. Long-term continuous therapy should be avoided where possible in infants & childn <12 yr. Elderly.
Adverse Reactions
Drug Interactions
Increased systemic exposure w/ CYP3A4 inhibitors eg, ritonavir, itraconazole.
