Azithromycin

Indications/Uses

Resp tract infections including otitis media, skin & soft-tissue infections & uncomplicated genital infections. Prophylaxis & as a component in the treatment regimen of Mycobacterium avium complex (MAC) infections. Prophylaxis of endocarditis in at risk patient unable to take penicillin. Management of trachoma & typhoid.

Dosage/Direction for Use

Adult 500 mg as a single daily dose for 3 days or initially 500 mg followed by 250 mg daily for a further 4 days. Chancroid & uncomplicated genital infections caused by Chlamydia trachomatis 1 g as a single dose. Uncomplicated gonorrhea 2 g as a single dose. Granuloma inguinale Initially 1 g followed by 500 mg daily or 1 g once wkly for at least 3 wk. Prophylaxis of disseminated MAC infections 1.2 g once wkly. Treatment or secondary prophylaxis of other antimycobacterial infections 500 mg once daily.

Administration

May be taken with or without food: May be taken w/ meals to reduce GI discomfort. Swallow whole.

Contraindications

Hypersensitivity to azithromycin, erythromycin, any other macrolide or ketolide antibiotic.

Special Precautions

May cause longer QT intervals. Hepatic & renal impairment; not to be given to patients w/ severe hepatic impairment.

GI disturbances. Headache, somnolence, taste disturbance.

Drug Interactions

May reduce the rate of absorption w/ antacids containing Al or Mg salts. Increased conc w/ nelfinavir. Theoretical possibility of ergotism w/ ergotamine derivatives. Possibility of increased digoxin levels. Elevated cyclosporine C_max & AUC_0-5. Increased phosphorylated zidovudine conc. Enhanced effect of astemizole, alfentanil. May increase QT interval prolongation, ventricular arrhythmias & Torsades de pointes w/ cisapride.
