Indications/Uses
Primary hypercholesterolemia, heterozygous familial hypercholesterolemia, HoFH as combined (mixed) hyperlipidemia in patients who have not responded adequately to diet & other appropriate measures.
Dosage/Direction for Use
Initially 10-20 mg once daily, may be adjusted at intervals of 4 wk up to max of 80 mg daily. Patient who requires large reduction in LDL-C Initially 40 mg daily.
Administration
May be taken with or without food: Avoid excessive consumption (>1 L/day) of grapefruit juice.
Contraindications
Hypersensitivity. Active liver disease or unexplained persistent elevations of serum transaminases. Women of childbearing potential not using adequate contraceptive measures. Pregnancy & lactation. Childn.
Special Precautions
Patients who consume substantial quantities of alcohol &/or have history of liver disease. Reported rhabdomyolysis w/ or w/o renal impairment; myalgia. Consider myopathy in any patient w/ diffuse myalgias, muscle tenderness or weakness, &/or marked elevation of creatinine phosphokinase. Increased risk of myopathy w/ concurrent administration of immunosuppressive drugs, including cyclosporine, fibric acid derivatives, nicotinic acid, azole antifungals or erythromycin. Use appropriate contraceptive measures during treatment in women of childbearing potential. Suspend treatment for the duration of pregnancy or until it has been determined that the woman is not pregnant. Women taking atorvastatin should not breastfeed.
Adverse Reactions
Diarrhea, constipation, flatulence, dyspepsia, abdominal pain, headache, nausea, myalgia, arthralgia, asthenia, insomnia, rash, muscle cramps, myositis, myopathy, paresthesia, peripheral neuropathy, pancreatitis, hepatitis, cholestatic jaundice, anorexia, vomiting, alopecia, pruritus, impotence, hyperglycemia, & hypoglycemia.
Drug Interactions
Risk of myopathy w/ other HMG-CoA reductase inhibitors. Decreased plasma conc w/ antacid; colestipol. Increased steady-state plasma digoxin conc. Increased AUC values of norethindrone & ethinylestradiol. Closely monitor patients receiving atorvastatin when combined w/ warfarin therapy. Increased plasma conc w/ erythromycin.
