Tetanus toxoid (adsorbed)
Indications/Uses
Prevention of tetanus in all age groups >6 wk following injury or animal bite, & neonatal tetanus in pregnant women. Persons who are at increased risk of attaining occupational or recreational injuries.
Dosage/Direction for Use
IM Primary immunization 2 doses of 0.5 mL inj in anteriolateral thigh in childn & in deltoid in adolescents & adults at least 4 wk apart, followed by 3rd dose 6-12 mth later. Prevention of neonatal tetanus Administer during pregnancy & should be given to the mother at 1st contact or as early as possible. Unimmunized women of childbearing age Give 2 additional booster doses, at least 1 yr after basic course of immunization w/ 3 doses. After injury or animal bite exposure Administer dose to patients w/ clean minor wounds & all other wounds but w/ unknown history of or <3 tetanus toxoid doses administered, or are immunodeficient; w/ clean minor wounds & had ≥3 doses of tetanus toxoid administered for >10 yr since last dose; w/ all other wounds & had ≥3 doses of tetanus toxoid administered for >5 yr since last dose.
Contraindications
Hypersensitivity. History of anaphylaxis or other serious allergic reactions following previous dose or any other tetanus toxoid containing vaccine. Defer immunization during course of any febrile illness (except mild URTI) or acute infection.
Special Precautions
Not to be administered in patients w/ history of severe arthus type hypersensitivity during previous dose, any bleeding disorders (eg, hemophilia or thrombocytopenia) or on anticoagulant therapy. Not to be given more than once every 10 yr for booster unless indicated by specific condition. Increased incidence of reactions to booster doses when given to previously immunized person. Ensure that inj does not enter into blood vessel. Observe vaccinee for 30-60 min after administration of vaccine for any immediate reactions. Administer antihistamine for mild allergic reactions & epinephrine for anaphylactic reactions. Pregnancy & impaired transplacental transfer of antitoxin in pregnant women w/ malaria. Lactation.
Adverse Reactions
Redness, warmth, edema, induration w/ or w/o tenderness, urticaria & rash; malaise, transient fever, pain, hypotension, nausea & arthralgia.
Drug Interactions
Immune response to vaccines may be reduced in patients who are receiving immunosuppressive therapies including radiation, corticosteroids, antimetabolites, alkylating agents & cytotoxic drugs. Caution in patients taking anticoagulant.
