Colchicine

Indications/Uses

Acute gout. Prophylaxis of recurrent gout. Prevention of acute attacks during initial therapy w/ allopurinol & uricosuric drugs.

Dosage/Direction for Use

Acute gout & pseudogout 1-2 tab every 2 hr until pain relief or nausea/vomiting/diarrhea occurs. Max: 4-8 mg. Acute gout prophylaxis 1 tab daily 1-4 times wkly to 1 tab daily. Pseudogout prophylaxis Up to 2 tab daily.

Overdosage

Administration

May be taken with or without food.

Contraindications

Hypersensitivity. Blood dyscrasias, uric acid kidney stones. Patients w/ renal or hepatic impairment taking P-gp or strong CYP3A4 inhibitors; undergoing haemodialysis. Pregnancy & lactation. Childn <2 yr.

Special Precautions

Discontinue use if nausea, vomiting, abdominal pain & diarrhea occur; signs & symptoms of blood cell dyscrasia eg, fever, stomatitis, sore throat, prolonged bleeding, bruising or skin disorders develop. Severe bone marrow depression eg, agranulocytosis, aplastic anaemia, thrombocytopenia. CV disease, GI disorders, patients w/ blood counts abnormalities. Check blood picture periodically. Not to be taken by patients w/ galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Colchicine-induced toxicity w/ co-administration of P-gp &/or moderate or strong CYP3A4 inhibitors. May affect ability to drive & use machines. Renal or hepatic impairment. Elderly & debilitated patients.

Drug Interactions

Avoid use w/ grapefruit & grapefruit juice. Concomitant use w/ P-gp or strong CYP3A4 inhibitors eg, clarithromycin, cyclosporine, erythromycin, verapamil & diltiazem, telithromycin, ketoconazole, itraconazole, HIV PIs, nefazodone.
