Pethidine HCl
Indications/Uses
Moderate to severe pain, obstet & pre-op analgesia, enhancement of anaesth, basal narcosis w/ phenothiazines.
Dosage/Direction for Use
SC/IM 25-100 mg 4 hrly. Obstet analgesia 50-100 mg every 1-3 hr if necessary. Max: 400 mg in 24 hr. Elderly & debilitated patient Max initial dose: 25 mg inj. Childn 0.5-2 mg/kg single IM dose, repeat if necessary but not >4 hrly. Slow IV 25-50 mg 4 hrly.
Contraindications
History of hypersensitivity or idiosyncratic response. Bronchial asthma, increased intracranial pressure, resp depression, hypotension, acute alcoholism. Concomitant use of MAOI w/in 14 days. Severe hepatic disease. Pregnancy & lactation.
Special Precautions
Not to be used in presence of phenobarb or any other enzyme-inducing agents. Decreased resp reserve; supraventricular tachycardia; head injuries, biliary tract disorders, hypothyroidism, adrenocortical & adrenal insufficiency, shock, prostatic hypertrophy; raised intracranial pressure or convulsive disorders; resp insufficiency; cancer or sickle anaemia; acute alcoholism. Tolerance, physical & psychological dependence. Avoid abrupt w/drawal. Prolonged & high-dose use. Cross-tolerance between narcotic analgesics can occur. Concomitant use w/ other CNS depressant drugs; phenothiazines; anticholinergics; benzodiazepines; serotonergic drugs. May affect ability to drive & use machines. Renal failure; severe renal & hepatic impairment. During labour. Not usually given pre-op to childn <1 yr. Newborn or premature infants. Elderly & debilitated patients.
Adverse Reactions
Constipation, resp depression, cough suppression, urinary retention, nausea, tolerance; mild euphoria, breath shortness, face edema, dizziness, vomiting, urticaria & other skin rashes, hypotension, CNS excitation.
Drug Interactions
Increased CNS & resp depression & hypotensive effect w/ alcohol, CNS depressants. Increased risk of resp depression, profound sedation, coma w/ benzodiazepines. Serotonin syndrome w/ serotonergic drugs. Incompatibility w/ barbiturate salts, aminophylline, amobarbital, floxacillin, furosemide, amylobarbitone Na, ephedrine sulphate, heparin Na, hydrocortisone Na succinate, methicillin Na, methylprednisolone Na succinate, morphine sulphate, nitrofurantoin Na, pentobarbitone Na, phenobarb Na, phenytoin Na, oxytetracycline HCl, thiamylal Na, thiopental Na, Na bicarbonate, sulphadiazine Na, Na iodide, sulphafurazole diethanolamine or thiopentone Na, acyclovir Na, imipenem, frusemide, idarubicin, allopurinol, amphotericin B cholesteryl sulphate complex, cefepime; K iodide-, aminosalicyclic acid- & salicylamide-containing soln. Colour changes or precipitation w/ minocycline HCl, tetracycline HCl, cefoperazone Na, mezlocillin Na, nafcillin Na, liposomal doxorubicin HCl.
