Amphotericin B (lipid complex)
Indications/Uses
Severe systemic &/or deep mycoses in cases where toxicity or renal failure precludes use of conventional amphotericin B in effective doses & where prior systemic antifungal therapy has failed. Disseminated candidiasis & aspergillosis; severely neutropenic patients.
Dosage/Direction for Use
Adult & childn 5 mg/kg single IV infusion at 2.5 mg/kg/hr.
Contraindications
Special Precautions
Not intended for common or superficial, clinically inapparent fungal infections diagnosed only by +ve skin or serological tests. Hyperkalaemia. Monitor renal function & serum K prior to & during therapy. Not to be diluted w/ saline soln or mix w/ other drugs or electrolytes. Not to be used w/ in-line filter. Concomitant use w/ hepatotoxic drugs; patients receiving nephrotoxic drugs; K supplementation in patients w/ previous hypokalaemia. Renal impairment, pre-existing renal disease. Pregnancy & lactation.
Adverse Reactions
Chills, fever, nausea, vomiting; increased serum creatinine, azotemia, hypokalaemia; hyperkalaemia; rigors, anaphylactoid reactions eg, hypotension, tachycardia, bronchospasm, dyspnoea, hypoxia, hyperventilation.
Drug Interactions
Closely monitor renal & haematologic function w/ concomitant use of zidovudine. Increased serum creatinine w/ high-dose cyclosporin. Enhanced toxicity of flucytosine. Concomitant use w/ antineoplastics, corticosteroids, ACTH, antiarrhythmics, digitalis glycosides, skeletal muscle relaxants. Monitor renal function w/ concomitant use of nephrotoxic drugs.
