Dosage/Direction for Use
Administration
Should be taken on an empty stomach: Dissolve effervescent tab in plain water, take at least 30 min before the 1st food/drink/medication of the day in a sitting/upright position. Do not lie down for at least 30 min after taking the oral soln. Do not swallow/chew/dissolve the tab in the mouth.
Contraindications
Hypersensitivity. Abnormalities of the oesophagus & other factors which delay oesophageal emptying eg, stricture or achalasia. Inability to stand or sit upright for at least 30 min. Hypocalcemia.
Special Precautions
Patients w/ active upper GI problems or history of GI diseases (w/in the last 1 yr). Discontinue treatment when oesophageal irritation occurs; consult w/ a doctor immediately. Consider the benefits & potential risks in patients w/ known Barrett's oesophagus. Risk of jaw osteonecrosis in cases of any history of cancer treatment, administration of bisphosphonate by IV route & dental disease; patients should maintain good oral hygiene. Consider causes of osteoporosis other than oestrogen deficiency & aging or glucocorticoid use. Correct/treat hypocalcaemia & other disorders affecting mineral metabolism prior to therapy. Monitor serum Ca & symptoms of hypocalcemia during therapy. Ensure adequate Ca & vit D intake in patients receiving glucocorticoids. Patients on a low Na diet. Not yet investigated in the treatment of glucocorticoid-induced osteoporosis. May affect ability to drive or operate machinery. Not recommended in patients w/ renal impairment (GFR <35 mL/min). Not to be used during pregnancy. Not to be used by breast-feeding women. Not recommended for use in childn <18 yr.
Adverse Reactions
Musculoskeletal pain (bone, muscle or joints) which is sometimes severe. Headache, dizziness, vertigo, abdominal pain & distension, dyspepsia, constipation, diarrhea, flatulence, oesophageal ulcer, dysphagia, acid regurgitation, alopecia, pruritus, joint swelling, asthenia, peripheral oedema.
Drug Interactions
Food & beverages (including mineral water), Ca supplement, antacids, & some oral medicinal products may interfere w/ alendronate absorption. Concomitant use w/ NSAIDs.
