Bupivacaine HCl 0.5% w/ adrenaline (1:200,000)
Indications/Uses
Anaesth techniques, including local infiltration, minor & major nerve blocks & epidural block.
Dosage/Direction for Use
Individualised dosage. Adult & childn >12 yr 400 mg over 24 hr. Lumbar epidural administration: Surgery & caesarean section 75-150 mg in 15-30 mL (5 mg/mL). Thoracic epidural administration: Surgery 25-50 mg in 5-10 mL (5 mg/mL). Caudal epidural block 100-150 mg in 20-30 mL (5 mg/mL). Major nerve block eg, brachial plexus, femoral, sciatic 50-175 mg in 10-35 mL (5 mg/mL). Field block eg, minor nerve blocks & infiltration ≤150 mg in ≤30 mL (5 mg/mL). Childn Should be calculated on a wt basis up to 2 mg/kg.
Contraindications
Hypersensitivity to amide type local anaesth & Na metabisulphite. IV regional anaesth (Bier's block) & for inj into inflamed or infected areas. Use in connection w/ anaesth in body areas supplied by end arteries (eg, penile block, Oberst block) or otherwise having compromised blood supply (eg, digits, nose, external ear, penis); in spinal anaesth; in obstet paracervical block. Thyrotoxicosis or severe heart disease (particularly when tachycardia is present).
Special Precautions
Not approved for IA continuous infusion. Repeat main dose aspiration prior to & during administration (inj slowly or in incremental doses at 25-50 mg/min rate) while closely observing patient's vital functions & maintaining verbal contact to avoid intravascular inj. Cardiac arrest. Acute toxicity effects on the CNS & CVS. Ventricular arrhythmia & fibrillation, sudden CV collapse. Elderly & patients in poor general condition. Patients w/ partial or complete heart block; w/ advanced liver disease or severe renal dysfunction; in late stages of pregnancy; treated w/ anti-arrhythmics class III. Epidural anaesth may lead to hypotension & bradycardia. Patients w/ epilepsy, impaired cardiac conduction, known drug sensitivities, hepatic or renal damage, liver disease or reduced liver blood flow. May temporarily impair locomotion & co-ordination.
Adverse Reactions
Hypotension; nausea. Paraesthesia, dizziness; bradycardia; HTN; vomiting; urinary retention.
Drug Interactions
Additive systemic toxic effects w/ lidocaine & mexiletin. Anti-arrhythmics class III eg, amiodarone. Severe & sustained hypotension or HTN w/ MAOI, TCA & phenothiazines. Severe, persistent HTN & possibly CVA & cardiac accidents when adrenaline-containing soln is concurrently used w/ oxytocic drugs of the ergot type. May oppose vasoconstrictor effects of adrenaline w/ neuroleptics eg, phenothiazines. Risk of serious cardiac arrhythmias w/ inhalation agents eg, halothane & enflurane. Enhanced pressor effects w/ nonselective β-blockers eg, propranolol.
