Nasonex

Symptomatic treatment of seasonal or perennial rhinitis in adults, adolescents & childn 2-11 yr; acute rhinosinusitis in patients ≥12 yr w/o signs or symptoms of bacterial infection. Relief of nasal congestion associated w/ seasonal allergic rhinitis in adults & ped patients ≥2 yr. Adjunctive treatment to antibiotics for acute episodes of sinusitis in adults & adolescents ≥12 yr. Nasal polyps & associated symptoms including congestion & loss of smell in adults ≥18 yr.

Seasonal or perennial rhinitis Adult (including geriatric patient) & adolescent 2 sprays in each nostril once daily. Maintenance: 1 spray in each nostril once symptoms are controlled. May be increased to max daily dose of 4 sprays in each nostril once daily if symptoms are inadequately controlled. Childn between 2 & 11 1 spray in each nostril once daily. Acute rhinosinusitis Patients ≥12 yr 2 actuations in each nostril bid. Nasal congestion associated w/ seasonal allergic rhinitis Adult & adolescent ≥12 yr 2 sprays in each nostril once daily. Childn 2-11 yr 1 spray in each nostril once daily. Adjunctive treatment of acute episodes of sinusitis Adult (including geriatric patient) & adolescent ≥12 yr 2 sprays in each nostril bid. May be increased to 4 sprays in each nostril bid if symptoms are inadequately controlled. Nasal polyposis Adult (including geriatric patient) & adolescent ≥18 yr 2 sprays in each nostril bid. May be reduced to 2 sprays in each nostril once daily once symptoms are adequately controlled.

Hypersensitivity.

Not to be used in presence of untreated localized infection involving nasal mucosa; patients who have experienced recent nasal surgery or trauma. Discontinue treatment if localized fungal infection of nose or pharynx develops or persistence of nasopharyngeal irritation occurs. Adrenal insufficiency from systemic corticosteroid w/drawal. Nasal septum perforation or increased IOP. Blurred vision or other visual disturbances. Signs or symptoms of severe bacterial infection eg, fever, persistent severe unilateral facial/tooth pain, orbital, or peri-orbital facial swelling, or worsening of symptoms after initial improvement. Patients w/ active or quiescent tuberculous infections of resp tract, or in untreated fungal, bacterial, systemic viral infections of ocular herpes simplex. Patients who are transferred from long-term use of systemic corticosteroids may experience systemic corticosteroid w/drawal resulting in adrenal insufficiency & unmask pre-existing allergic condition eg, allergic conjunctivitis & eczema. Potentially immunosuppressed patients at risk of exposure to certain infections eg, chickenpox, measles. Periodically examine patients in long-term treatment for possible changes in nasal mucosa. Women of childbearing age. Pregnancy & lactation. Observe infants born of mothers who received corticosteroids during pregnancy for hypoadrenalism. Childn & adolescents <18 yr in treatment of nasal polyposis; childn <12 yr in treating symptoms of rhinosinusitis.

Headache, epistaxis (ie, frank bleeding, blood-tinged mucus, & blood flecks), pharyngitis, nasal burning, irritation & ulceration, sinus headache. Blurred vision.

Increased plasma conc & risk of side effects w/ strong CYP3A4 inhibitors eg, ketoconazole, itraconazole, clarithromycin, ritonavir, cobicistat-containing products.
