Indications/Uses
Infections of the urinary tract including cystitis, urethritis, & pyelonephritis or genitals (for 500 mg) eg, gonorrhea, chlamydia, chancroid; resp tract including sinusitis, bronchitis, & pneumonia; GIT including diarrhea caused by bacterial pathogens eg, Shigella, Salmonella, Vibrio parahemolytica; bone & joint including osteomyelitis caused by susceptible bacteria; skin & soft tissue. Septicemia, meningitis, peritonitis. 250 mg: Infected wounds. Prevention of lower resp infections in immunocompromised patients; neutropenic infections in immunosuppressed patients (lower than normal WBC).
Dosage/Direction for Use
Adult Recommended dose: 250-750 mg bid every 12 hr. Max daily dose: 1.5 g. 250 mg Cystitis ½ - 1 tab bid. Infections of resp tract, urinary tract, kidney & GIT (including typhoid fever) ½ tab bid. Other infections 2 tab bid. Severe infections May be increased to 3 tab bid. Acute gonorrhea including uncomplicated acute cystitis 1 tab once daily. Duration of treatment: Usually given for 5-10 days depending on the severity of the disease. Continue for at least 3 days after fever subsides or symptoms disappear. Patient w/ CrCl <20 mL/min Normal dosage once daily. Reduce to ½ dosage if bid. 500 mg UTI 250-500 mg bid every 12 hr for 7-14 days. Gonorrhea caused by Neisseria gonorrhoeae 250-500 mg once daily. Digestive tract infectious disease 500 mg bid every 12 hr for 5-7 days. Typhoid fever 500 mg bid every 12 hr for 10 consecutive days. Lower resp tract, skin & soft tissue, bone & joint infection 500-750 mg bid every 12 hr for 7-14 days. Bone infection May take up to 4-6 wk.
Administration
May be taken with or without food: Take w/ at least 1 glass of water (240 mL) & 1-2 hr before or after antacids.
Contraindications
250 mg: Hypersensitivity to ciprofloxacin or other fluoroquinolones. Pregnancy & lactation. Childn & adolescents. 500 mg: Hypersensitivity to quinolones.
Special Precautions
Discontinue use if rash, joint or muscle pain, wrists or ankles tendon pain; blood system abnormalities including bone marrow suppression occur. SJS, TEN, erythema multiforme. Diabetic patients. History of epilepsy or other CNS disorders prone to seizures. Concomitant use w/ theophylline, methylxanthine, caffeine. Hepatic & renal impairment. 250 mg: History of allergic reactions to other quinolone antibiotics. Avoid prolonged sunlight or UV exposure. 500 mg: Hypersensitivity. May cause nausea, vomiting, dizziness, headache, nervous system stimulation, kidney inflammation, tendonitis or tendon rupture, skin sensitivity to sunlight & severe allergic reactions.
Adverse Reactions
Nausea, vomiting, diarrhea, abdominal pain, headache, dizziness & rash.
Drug Interactions
May interfere w/ drug metabolism & increase plasma levels of theophylline, methylxanthine, caffeine. Increased effects of warfarin. Reduced absorption w/ Al- & Mg-containing antacids, sucralfate, Fe- or Zn-. containing drugs. Drugs that cause alkalized urine eg,citrates, bicarbonate of soda.
