Valacyclovir HCl
Indications/Uses
Shingles (herpes zoster), herpes simplex infection of the skin & mucous membranes including initial & recurrent genital herpes. Prophylaxis of recurrent herpes simplex infections of the skin & mucous membranes including genital herpes.
Dosage/Direction for Use
Adult Shingles (herpes zoster) 1,000 mg tds for 7 days. Herpes simplex infections 500 mg bd for 5 days (recurrent infections) & 10 days (severe initial episodes). Prophylaxis of recurrences of herpes simplex infections Immunocompetent: 500 mg once daily. Immunocompromised: 500 mg bd. Renal impairment Herpes zoster CrCl at least 50 mL/min 1 g tds, 30-49 mL/min 1 g bd, 10-29 mL/min 1 g once daily, <10 mL/min 500 mg once daily. Herpes simplex CrCl at least 30 mL/min 500 mg bd, <30 mL/min 500 mg once daily. Herpes simplex prevention Adult Immunocompetent: CrCl at least 30 mL/min 500 mg once daily, <30 mL/min 250 mg once daily. Immunocompromised: CrCl at least 30 mL/min 500 mg bd, <30 mL/min 500 mg once daily. Reduction of genital herpes transmission CrCl at least 30 mL/min 500 mg once daily, <30 mL/min 250 mg once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to valacyclovir or acyclovir.
Special Precautions
Advise patients to closely monitor for skin reactions. Immediately w/draw & consider an alternative treatment if signs & symptoms suggestive of DRESS appear; do not restart treatment if patient develops DRESS. Ensure adequate fluid intake in patients who are at risk of dehydration. Advise patients to use safer sex practices to reduce risk of transmitting genital herpes. Renal impairment. Use of higher doses (>4 g/day) in hepatic impairment & liver transplantation. Pregnancy & lactation. Childn. Elderly.
Adverse Reactions
Headache; nausea.
Drug Interactions
May increase blood conc w/ other drugs excreted via active renal tubular secretion. Increased AUC w/ cimetidine & probenecid. Potentially increased blood levels of valacyclovir & concurrent medicinal products which compete w/ acyclovir for elimination. Increased AUC of acyclovir & inactive metabolite of mycophenolate mofetil. Concomitant use w/ medicinal products affecting other aspects of renal function (eg, ciclosporin, tacrolimus).
