Tiotropium Br monohydrate
Indications/Uses
Maintenance bronchodilator treatment to relieve COPD symptoms. Add-on maintenance bronchodilator treatment in patients ≥6 yr w/ moderate to severe asthma.
Dosage/Direction for Use
Adult & elderly 2 puffs once daily at the same time of the day. Childn 6-17 yr Asthma 2 puffs once daily at the same time each day.
Contraindications
Hypersensitivity to tiotropium Br, atropine or its derivatives (eg, ipratropium or oxitropium).
Special Precautions
Immediate hypersensitivity reactions may occur. Not for the initial treatment of acute episodes of bronchospasm or relief of acute symptoms. Not to be used as 1st-line monotherapy for asthma. Patients w/ narrow-angle glaucoma, prostatic hyperplasia or bladder-neck obstruction; recent MI <6 mth, any unstable or life-threatening cardiac arrhythmia or cardiac arrhythmia requiring intervention or change in drug therapy in the past yr, hospitalisation of heart failure (NYHA class III or IV) w/in the past yr; known cardiac rhythm disorders. May cause inhalation-induced bronchospasm. Avoid contact w/ eyes; discontinue use if precipitation or worsening of narrow-angle glaucoma, eye pain or discomfort, temporary vision blurring, visual halos or coloured images in association w/ red eyes from conjunctival congestion & corneal oedema occur. Possible dry mouth associated w/ dental caries (long-term use). Not to be used more frequently than once daily. Contains benzalkonium Cl which may cause wheezing & breathing difficulties; increased risk for adverse events in patients w/ asthma. Dizziness or blurred vision may influence ability to drive & use machinery. Moderate to severe renal impairment (CrCl ≤50 mL/min). Avoid use during pregnancy. Lactation. Not recommended for COPD in childn & adolescents <18 yr. Childn <1 yr for asthma.
Drug Interactions
Not recommended to be co-administered w/ other anticholinergic-containing drugs.
