Adult Prevention of vit D deficiency 25,000 IU (1 amp)/mth. Treatment of vit D deficiency (<25 ng/mL) 50,000 IU (2 amp)/wk for 6-8 wk, followed by maintenance therapy (1,400-2,000 IU daily may be required). Adjunctive therapy for osteoporosis 25,000 IU (1 amp)/mth. Paed Prevention of vit D deficiency 1-18 yr 25,000 IU (1 amp) every 6 wk, 0-1 yr 25,000 IU (1 amp) every 8 wk. Treatment of vit D deficiency 0-18 yr 25,000 IU (1 amp) once every 2 wk for 6 wk, followed by maintenance therapy (400-1,000 IU daily).
Patients receiving treatment for CV disease. Monitor serum & urinary Ca levels in patients w/ sarcoidosis. Allowances should be made for the total dose of vit D. Risk of renal stones w/ concomitant Ca supplementation. Monitor Ca & phosphate levels in patients w/ renal impairment. Not to be used in combination w/ Ca in patients w/ severe renal impairment. Pregnancy (max: 2,000 IU daily) & lactation. Increased risk of fractures in elderly w/ greatest increase occurring during 1st 3 mth after dosing.
May reduce effect of vit D3 w/ anticonvulsants (eg, phenytoin) or barbiturates & possibly other hepatic enzymes-inducing drugs. Decreased urinary elimination of Ca w/ thiazide diuretics. May decrease effect w/ glucocorticoids. May increase risk of digitalis toxicity (arrhythmia) w/ drugs containing digitalis & other cardiac glycosides. May reduce GI absorption w/ ion exchange resin (eg, cholestyramine, colestipol HCl, orlistat) or laxative (eg, paraffin oil). Actinomycin & imidazole antifungal agents may interfere w/ vit D activity.
