Cefuroxime axetil
Indications/Uses
URTI, lower resp tract, GUT, skin & soft tissue infections; gonorrhoea, acute uncomplicated gonococcal urethritis & cervicitis; early Lyme disease & subsequent prevention of late Lyme disease.
Dosage/Direction for Use
Usual course of therapy: 7 days (range: 5-10 days). Adult Most infections, UTI, mild to moderate lower resp tract infections pyelonephritis 250 mg bid. More severe lower resp tract infections or suspected pneumonia 500 mg bid. Uncomplicated gonorrhoea 1 g single dose. Adult & childn >12 yr Lyme disease 500 mg bid for 14 days (range: 10-21 days). Sequential therapy: Pneumonia Cefuroxime Na 1.5 g tid or bid (IV/IM) for 48-72 hr followed by cefuroxime axetil 500 mg bid for 7-10 days, Acute exacerbations of COPD Cefuroxime Na 750 mg tid or bid (IV/IM) for 48-72 hr followed by cefuroxime axetil 500 mg bid for 7-10 days. Renal impairment CrCl ≥30 mL/min 125-500 mg bid, 10-29 mL/min Standard individual dose every 24 hr, <10 mL/min Standard individual dose every 48 hr, during haemodialysis Additional single standard individual dose at end of each dialysis. Oral susp Infant & childn 3 mth-12 yr Most infections 10 mg/kg bid to max of 250 mg daily. Otitis media or more severe infections 15 mg/kg bid to max of 500 mg daily. Lyme disease 15 mg/kg bid to max of 500 mg daily for 14 days.
Administration
Should be taken with food: Take immediately after meals for optimum absorption.
Contraindications
Hypersensitivity to cephalosporins.
Special Precautions
Hypersensitivity to penicillins or other β-lactams. Discontinue use if prolonged or significant diarrhoea occurs or patient experiences abdominal cramps. May result in overgrowth of Candida, & in prolonged use, of other non-susceptible organisms eg, enterococci & Clostridium difficile, Pseudomembranous colitis. Jarisch-Herxheimer reaction following treatment of Lyme disease. May cause dizziness which may affect driving or operating machinery. Pregnancy & lactation.
Adverse Reactions
Overgrowth of Candida; eosinophilia; headache, dizziness; GI disturbances including diarrhoea, nausea, abdominal pain; transient increases of hepatic enzyme levels [ALT (SGPT), AST (SGOT), LDH].
Drug Interactions
Bioavailability may be lowered w/ drugs reducing gastric acidity. Lowered reabsorption of oestrogen & reduced efficacy of combined OCs. False -ve result may occur in ferricyanide test.
