Indications/Uses
Treatment or prevention of bronchospasm. Syr: Relief of bronchospasm in bronchial asthma of all types, chronic bronchitis & emphysema. Nebules: Routine management of chronic bronchospasm (unresponsive to conventional therapy) & treatment of acute severe asthma (status asthmaticus).
Dosage/Direction for Use
Syr Adult Usual effective dose: 10 mL tid or qid, may be gradually increased to 20 mL if adequate bronchodilation is not obtained. Childn >12 yr 5-10 mL tid or qid, 6-12 yr 5 mL tid or qid, 2-6 yr 2.5-5 mL tid or qid. Elderly or those unusually sensitive to β-adrenergic stimulant drug 5 mL tid or qid. MDI Relief of acute bronchospasm Adult 100 or 200 mcg. Childn 100 mcg. May be increased to 200 mcg if required. Prevention of allergen or exercise-induced bronchospasm Adult 200 mcg before exertion. Childn 100 mcg before exertion. May be increased to 200 mcg if required. Chronic therapy Adult & childn Up to 200 mcg qid. Nebules Adult & childn Initially 2.5 mg by wet inhalation, may be increased to 5 mg & may be repeated qid. Severe airway obstruction Adult 40 mg daily w/ strict medical supervision.
Administration
Should be taken on an empty stomach: Take 1 hr before or 2 hr after meals.
Contraindications
Hypersensitivity. Not to be used to arrest uncomplicated premature labour or threatened abortion.
Special Precautions
Sudden & progressive deterioration in asthma control. Thyrotoxicosis. Potentially serious hypokalaemia may result from nebulised therapy; caution in acute severe asthma as effect may be potentiated by concomitant use w/ xanthine derivatives, steroids, diuretics & by hypoxia; monitor serum K levels. Paradoxical bronchospasm may occur in inhalation therapy. Pregnancy. Not recommended during lactation. Syr: Increased risk of dental caries in long-term treatment. Nebules: Risk of anoxia through hypoventilation. Transient hypoxaemia may occur. Must be used by inhalation, w/ a nebuliser & should be breathed through the mouth. Must not be inj or swallowed. Patients known to receive large doses of other sympathomimetic drugs. Concomitant use w/ nebulised anticholinergics (ipratropium Br). Avoid contact w/ eyes. Monitor for the development of elevated serum lactate & consequent metabolic acidosis. Infant <18 mth. Syr/Nebules: Induced reversible metabolic changes (eg, increased blood sugar levels) & ketoacidosis may develop in diabetic patient. Concurrent use w/ corticosteroids.
Adverse Reactions
Tremor, headache, tachycardia. Syr: Palpitations, muscle cramps.
Drug Interactions
Not to be prescribed w/ non-selective β-blockers eg, propranolol.
