Indications/Uses
Serious bacterial infections caused by susceptible strains of β-lactamase producing organisms in lower resp tract infections; complicated & uncomplicated UTI; intra-abdominal infections; skin & structure infections; bacterial septicemia; gynecological infections.
Dosage/Direction for Use
IV infusion over 30 min. Adult 3.375 g every 6 hr. Nosocomial pneumonia Piperacillin 16 g + tazobactam 2 g. Continue treatment w/ aminoglycoside if Pseudomonas aeruginosa isolated. To be given 7-14 days. Community-acquired pneumonia, intra-abdominal infections (appendicitis &/or peritonitis), uncomplicated & complicated UTI, skin & skin structure infections, pelvic inflammatory disease Childn >40 kg 4.5 g every 6 hr, ≥9 mth or up to 40 kg Piperacillin 100 mg + tazobactam 12.5 mg per kg every 8 hr or 3.375 g every 6 hr + aminoglycoside, 2 mth & 9 mth Piperacillin 80 mg + tazobactam 10 mg per kg every 8 hr. Renal impairment CrCl >40 mL/min 3.375 g every 6 hr, 20-40 mL/min 2.25 g every 6 hr, <20 mL/min & patient not receiving dialysis 2.25 g every 8 hr. Patient on hemodialysis 2.25 g every 12 hr. Additional dose of 0.75 g should be administered following each dialysis period on hemodialysis days. Patient on CAPD 2.25 g every 12 hr or 8 hr. Nosocomial pneumonia CrCl >40 mL/min 4.5 g every 6 hr, 20-40 mL/min 3.375 g every 6 hr, <20 mL/min & patient not receiving dialysis 2.25 g every 6 hr. Patient on hemodialysis 2.25 g every 8 hr. Additional dose of 0.75 g should be administered following each dialysis period on hemodialysis days.
Contraindications
Hypersensitivity to penicillins, cephalosporin, β-lactamase antibiotics, tetracyclines or β-lactamase inhibitors.
Special Precautions
Cystic fibrosis, bowel inflammation, bleeding problems, CHF; history of severe diarrhea or bowel problems due to antibiotics. Patients on salt-restricted diet or w/ low blood K levels. Concomitant use w/ prescription/non-prescription medicine, herbal prep or dietary supplement. Allergies to medicines, food or other substances. Patients w/ kidney problems & on dialysis. Pregnancy & lactation.
Adverse Reactions
Agitation; constipation; diarrhea; dizziness; headache; indigestion; nausea; pain; swelling or redness at inj site; sleeplessness; vomiting. Severe allergic reactions (rash; hives; itching; difficulty breathing; chest tightness; swelling of the mouth, face, lips or tongue); bloody stools; chest pain; decreased urination; depression; fainting; fast, slow or irregular heartbeat; fever, chills or sore throat; hallucination; inflammation at inj site; prolonged muscle relaxation; red, swollen or blistered skin; seizure; severe diarrhea, vomiting or stomach pain; shortness of breath; swelling of the hands, ankles or feet; tremor; unusual tiredness or weakness; vag irritation or discharge; vein inflammation or tenderness; yellowing of the eyes or skin.
Drug Interactions
Decreased effectiveness of aminoglycosides (eg, tobramycin) or OCs. Decreased effectiveness & increased risk of side effects of anticoagulants (eg, warfarin). Increased risk of side effects of chemotherapeutic agents or diuretics (eg, furosemide, hydrochlorothiazide); heparin, methotrexate or nondepolarizing muscle relaxants (eg, vecuronium). Decreased effectiveness & increased risk of side effects w/ tetracyclines (eg, doxycycline).
