Indications/Uses

Sinus bradycardia. Adjunct in reversal of neuromuscular blockade.

Dosage/Direction for Use

Sinus bradycardia/CPR Premed: Adult 0.4-1 mg IV/IM/SC repeat every 3-5 min. Max: 40 mcg/kg. Childn 10-20 mcg/kg IV/IM/SC to be repeated as needed every 2 hr. Min: 0.1 mg. Adjunct in reversal of neuromuscular blockade 0.6-1.2 mg IV for each neostigmine 0.5-2.5 mg, pyridostigmine 10-20 mg or edrophonium 10-20 mg.

Contraindications

Hypersensitivity to atropine or other anticholinergics. Patients w/ obstructive disease of the GIT (eg, pyloroduodenal stenosis), cardiospasm, paralytic ileus or intestinal atony (geriatric or debilitated patients). Megacolon complicating ulcerative colitis. Prostatic enlargement. Unstable CV status in acute hemorrhage. Tachycardia secondary to cardiac insufficiency of thyrotoxicosis. Toxemia of pregnancy. Obstructive uropathy (eg, bladder neck obstruction caused by prostatic hypertrophy). Closed angle glaucoma. Childn w/ high ambient temp.

Special Precautions

Patients >40 yr particularly w/ severe heart disease, HTN, ulcerative colitis, ileus, chronic lung disease, hyperthyroidism, autonomic neuropathy, hepatic or renal disease or prostatic hypertrophy, esophageal reflux or hiatus hernia. Avoid abrupt change in dose. Debilitated patients. Chronic pulmonary disease. May precipitate acute glaucoma in susceptible patients. Myasthenia gravis; cardiac insufficiency or failure, MI or ischemia. Development of angina in patients w/ cardiac problems; arrhythmias in adult & ped. Development of ventricular fibrillation. Pregnancy & lactation. Childn & infants. Elderly.

Adverse Reactions

Tachycardia & palpitations; dry mouth, thirst; urinary difficulty & retention, constipation; mydriasis w/ cycloplegia; photophobia; flushing & dry skin.

Drug Interactions

Enhanced effects w/ amantadine, some antihistamines, phenothiazine antipsychotics & TCAs. May enhance the effects of antimuscarinics w/ MAOIs. May affect the absorption of drugs w/ antimuscarinics by reduction in gastric motility. May antagonize GI effects of cisapride, domperidone & metoclopramide. Antagonism w/ parasympathomimetics.
