Indications/Uses

HTN as monotherapy or in combination w/ other antihypertensive agents. Telzyd 20/Telzyd 80 Reduction of the risk of MI, stroke, or death from CV causes in patients ≥55 yr at high risk of developing major CV events who are unable to take ACE inhibitors.

Dosage/Direction for Use

Telzyd 20/Telzyd 80 HTN Individualized dosage. Initially 40 mg once daily. CV risk reduction 80 mg once daily. Telzyd 40 Initially 40 mg once daily. Max: 80 mg once daily.

Overdosage

Administration

May be taken with or without food.

Contraindications

Telzyd 20/Telzyd 80 Hypersensitivity. Telzyd 40 Hypersensitivity. Biliary obstructive disorders. Concomitant use w/ aliskiren-containing products in patients w/ DM or renal impairment (GFR <60 mL/min/1.73 m²). Severe hepatic impairment. Pregnancy (2nd or 3rd trimester).

Special Precautions

Dual blockade of RAAS. Hyperkalemia. Pregnancy & lactation. Childn. Telzyd 20/Telzyd 80 Hypotension; hyperkalemia, impaired renal function. Telzyd 40 Hepatic impairment; renovascular HTN; renal & kidney transplantation; intravascular hypovolaemia; primary aldosteronism; aortic & mitral valve stenosis, obstructive hypertrophic cardiomyopathy; diabetic patients treated w/ insulin or antidiabetics. Galactose intolerance, total lactase deficiency or glucose-galactose malabsorption. Black hypertensive population.

Adverse Reactions

Telzyd 20/Telzyd 80 GI disturbances, pharyngitis, back pain. Decrease in Hb (rarely), increase in uric acid & urticaria. Telzyd 40 Anaphylactic reaction & angioedema; acute renal failure.

Drug Interactions

Increase plasma conc of digoxin. Increase conc & toxicity of lithium. Decreased C_max & AUC w/ ramipril & ramiprilat. Telzyd 20/Telzyd 80 Acetaminophen, amlodipine, glyburide, simvastatin, hydrochlorothiazide, warfarin, or ibuprofen. Telzyd 40 Increase serum K w/ K sparing diuretics eg, spironolactone, eplerenone, triamterene, or amiloride, K supplements, or K-containing salt substitutes. NSAIDs. Thiazide or loop diuretics. Dual blockade RAAS w/ ACE-inhibitors, angiotensin II receptor blockers or aliskiren. Potentiated hypotensive effects w/ all antihypertensive including telmisartan; baclofen, amifostine. Aggravated hypotension w/ alcohol, barbiturates, narcotics or antidepressants. Reduced antihypertensive effect w/ systemic use corticosteroids.
