Tetanus toxoid (adsorbed)
Indications/Uses
Prevention of neonatal tetanus by immunizing women of childbearing age, & tetanus in infants, childn & adults especially those exposed to tetanus infection following injury & those engaged in outdoor activities.
Dosage/Direction for Use
IM Deltoid muscle in women & older childn; anterolateral upper thigh in younger childn. Primary immunization 2 dose of 0.5 mL at least 4 wk apart, followed by 3rd dose 6-12 mth later. Booster dose of 0.5 mL is recommended at every feasible interval (5-10 yr for adult. Prevention of neonatal tetanus Previously unimmunized women of childbearing age 5-dose schedule: After basic course w/ 3 dose, 2 additional booster dose at least 1 yr after previous dose or during subsequent pregnancy. Infection w/ injury or other cause >5 yr elapsed after primary or booster dose: 0.5 mL. Inadequate immunization history: 1,500 IU tetanus antiserum + 0.5 mL toxoid in separate syringe administered to different body site. 2nd 0.5 mL dose of toxoid after 2 wk & 3rd dose after further 1 mth is recommended.
Contraindications
Persons w/ severe reaction to previous dose of tetanus toxoid. Defer immunization during the course of any febrile illness or acute infection.
Special Precautions
Ensure that inj does not enter a blood vessel. Observe vaccinees for ≥30 min for possible immediate or early allergic reactions. Use adrenaline inj at the 1st suspicion of anaphylaxis. Hydrocortisone & antihistamine should be available in addition to supportive measures eg, O2 inhalation. Increased incidence of local & systemic reactions to booster doses when given to previously immunized persons. Caution w/ IM inj in patients on anticoagulant therapy.
Adverse Reactions
Inflammation w/ transient fever, malaise & irritability.
Drug Interactions
Reduced immune response w/ immunosuppressive therapy.
