Butorphanol tartrate
Indications/Uses
Relief of moderate to severe pain. For pre-op or pre-anesth medication; as a supplement to balanced anesth. For relief of pain during labor.
Dosage/Direction for Use
Pain IV 1 mg repeated every 3-4 hr as necessary. Effective dosage range: 0.5-2 mg repeated every 3-4 hr depending on the severity of pain. IM 2 mg in patients who will be able to remain recumbent. May be repeated every 3-4 hr in the event drowsiness or dizziness occurs. Elderly, patient w/ hepatic or renal impairment ½ the recommended adult dose (0.5 mg IV or 1 mg IM) repeated depending on patient's response, but not <6 hr apart. Pre-op, pre-anesth medication 2 mg IM, administered 60-90 min before surgery. Balanced anesth 2 mg IV shortly before induction &/or 0.5-1 mg IV increments during anesth. The increment may be higher, up to 0.06 mg/kg (4 mg/70 kg), depending on previous sedative, analgesic & hypnotic drugs administered. Labor Initially 1 or 2 mg IV or IM repeated after 4 hr in patients at full term or w/o signs of fetal distress in early labor.
Contraindications
Patients dependent on narcotics; patients taking opioid analgesics chronically.
Special Precautions
Concomitant use of opioids w/ CNS depressants. Patients who have recently received repeated doses of narcotic analgesic medication; head injury; acute MI, ventricular dysfunction or coronary insufficiency. Resp depression may be produced especially in patients receiving other CNS active agents or patients suffering from CNS diseases or resp impairment. Discontinue in case of severe HTN. Do not consume alcohol. Concomitant use w/ drugs having antagonistic action & effect of opioid less frequent than morphine but can be abused. Patients w/ history of drug abuse or patients receiving the drug on a continuous basis for an extended period. Drowsiness & dizziness may occur. May impair the mental & physical abilities required for the performance of potentially dangerous tasks eg, driving a car or operating machinery. Pregnancy & lactation. Elderly >65 yr. Childn <18 yr.
Adverse Reactions
Somnolence, dizziness, nausea &/or vomiting.
Drug Interactions
Increased CNS depressant effects w/ CNS depressants (eg, alcohol, barbiturates, tranquilizers & antihistamines). Caution in concomitant use w/ medications that affect hepatic metabolism of drugs (eg, cimetidine, erythromycin, theophylline).
