Indications/Uses
Adjunct therapy in adult w/ partial seizures. Generalized anxiety disorder (GAD) in adult.
Dosage/Direction for Use
150-600 mg/day in 2 or 3 divided doses. Epilepsy Initially 150 mg/day given as 2 or 3 divided doses. May be increased to 300 mg/day after 1 wk. Max: 600 mg/day after an additional wk. GAD Initially 150-600 mg/day given as 2 or 3 divided doses. May be increased to 300 mg/day after 1 wk then 450 mg/day following an additional wk. Max: 600 mg/day after an additional wk. Renal impairment CrCl ≥60 mL/min 150 mg bid. Max: 600 mg/day. 30-60 mL/min 75 mg once daily or bid. Max: 300 mg/day. 15-30 mL/min 25-50 mg once daily or bid. Max: 150 mg/day. <15 mL/min 25 mg once daily. Max: 75 mg/day. Supplementary dosage following hemodialysis 25 mg as single dose. Max: 100 mg/day.
Administration
May be taken with or without food.
Special Precautions
Adjust hypoglycemic medications in diabetic patients who gain wt during treatment. Discontinue immediately if symptoms of angioedema occur; serious skin reactions. Associated w/ dizziness & somnolence which could increase the occurrence of accidental injury (fall). Loss of consciousness, confusion, mental impairment; transient visual blurring & other changes in visual acuity. W/drawal symptoms have been observed after discontinuation; taper the dose gradually over a min of 1 wk. CHF. Evaluate patients for history of drug abuse & observe for signs of misuse or abuse (eg, development of tolerance, dose escalation, drug-seeking behavior). Patients w/ severe CHF. Concomitant use w/ opioid analgesics. History of substance abuse. Discontinue if myopathy is diagnosed or suspected or if markedly elevated creatine kinase levels occurs. Encephalopathy. Renal failure. May affect the ability to drive & use machines. Effective contraception in women of child bearing potential. Pregnancy & lactation. Childn. Elderly >65 yr.
Adverse Reactions
Dizziness, somnolence, headache. Nasopharyngitis; increased appetite; euphoric mood, confusion, irritability, disorientation, insomnia, decreased libido; ataxia, abnormal coordination, tremor, dysarthria, amnesia, memory impairment, disturbance in attention, paraesthesia, hypoaesthesia, sedation, balance disorder, lethargy; blurred vision, diplopia; vertigo; vomiting, nausea, constipation, diarrhoea, flatulence, abdominal distension, dry mouth; muscle cramp, arthralgia, back pain, pain in limb, cervical spasm; erectile dysfunction; peripheral oedema, edema, abnormal gait, fall, feeling drunk, feeling abnormal, fatigue; increased wt.
Drug Interactions
Increased risk of developing angioedema w/ ACE inhibitor. Decreased absorption w/ gabapentin. May potentiate the effects of ethanol & lorazepam. Resp failure & coma w/ other CNS depressants. Additive in the impairment of cognitive & gross motor function caused by oxycodone. Concomitant use w/ thiazolidinedione may cause wt gain &/or fluid retention.
