Paracetamol
Indications/Uses
Tab/120 mg/5 mL syr: Relief of minor aches & pains eg, headache, backache, menstrual cramps, muscular aches, arthritis, toothache & pain associated w/ common cold & flu; fever. 250 mg/5 mL syr: Minor febrile conditions, reduction of temp & relief of headache & joint muscle pains. Supp: Relief of fever & mild to moderate pain.
Dosage/Direction for Use
Tab Adult & childn ≥12 yr 1-2 tab every 4-6 hr. 120 mg/5 mL syr Childn 6-12 yr 10-20 mL tid-qid, 1-5 yr 5-10 mL tid-qid. 250 mg/5 mL syr Adult 15 mL tid-qid. Childn 7-12 yr 5-10 mL tid-qid, 2-6 yr 2.5-5 mL tid-qid. Supp Childn 7-12 yr 1-2 (250 mg) supp every 4 hr as needed qid, 3-6 yr 1 (250 mg) supp or 1-2 (125 mg) supp every 4 hr as needed qid, 1-2 yr 1 (125 mg) supp every 4 hr as needed qid.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Long-term use in patients w/ anemia or cardiac, pulmonary, renal or hepatic disease. Ulceration or inflammation of GIT. Supp: Concomitant use w/ other medicine containing paracetamol.
Special Precautions
Patients w/ liver disease; kidney impairment; alcohol dependence. Concomitant use w/ warfarin & other drugs affecting the liver. Fever >3 days & pain continued for >5 days. Supp: Patient w/ G6PD deficiency.
Adverse Reactions
Skin rashes, hypersensitivity reactions, changes in number of WBC & platelets, minor stomach & intestinal disturbance. Supp: redness of the rectal mucous membranes.
Drug Interactions
May increase anticoagulant effect of warfarin. Absorption may be increased by metoclopramide or domperidone & reduced by cholestyramine. Increased susceptibility to harmful effects to the liver w/ enzyme stimulating medicines. Syr: Alcohol, chloramphenicol, aspirin, phenobarb, liver enzyme inducers, hepatotoxic agents. Supp: Increased risk of toxic effects w/ other painkillers in prolonged use; chronic intake of alcohol. Reduces efficacy w/ oral contraceptive. Increased risk of hypothermia w/ phenothiazines. Increases exposure to chloramphenicol. Probenecid may affect elimination.
