Management of renal tubular acidosis w/ Ca stones, hypocitraturic Ca oxalate nephrolithiasis of any etiology, & uric acid lithiasis w/ or w/o Ca stones.

Administration
Should be taken with food: Take 30 min after meals. Swallow whole, do not crush/chew/suck.
Contraindications
Patients w/ hyperkalemia or conditions predisposing to hyperkalemia eg, chronic renal failure, uncontrolled DM, acute dehydration, strenuous physical exercise in unconditioned individuals, adrenal insufficiency, extensive tissue breakdown, or administration of K-sparing agent (eg, triamterene, spironolactone or amiloride). Patients suffering from delayed gastric emptying, esophageal compression, intestinal obstruction or stricture, or taking anticholinergic medication. Not to be given to patients w/ PUD. Renal insufficiency (GFR <0.7 mL/kg/min).
Hyperkalemia & cardiac arrest in patients w/ impaired mechanisms for excreting K. Potentially fatal hyperkalemia can develop rapidly & be asymptomatic. Avoid in patients w/ chronic renal failure, or other condition which impairs K excretion eg, severe myocardial damage or heart failure. Avoid concomitant administration w/ K-sparing diuretics (eg, triamterene, spironolactone or amiloride). Discontinue immediately & investigate possibility of bowel perforation or obstruction if there is severe vomiting, abdominal pain or GI bleeding. Patient taking both diuretics & digitalis prep. Use 24 hr urinary citrate &/or pH measurements to determine adequacy of initial dosage & to evaluate effectiveness of any dosage change. Measure urinary citrate &/or pH every 4 mth. Pregnancy & lactation. Childn.
