Indications/Uses
Prevention of nausea & vomiting associated w/ initial & repeat courses of emetogenic cancer chemotherapy, including high-dose cisplatin. Prevention & treatment of post-op nausea & vomiting (PONV).
Dosage/Direction for Use
Tab Adult 8 mg up to 2 hr before treatment followed by 8 mg to 12 hr later. Delayed emesis 8 mg bid for up to 5 days after the end of a course of chemotherapy. Childn 4-11 yr 4 mg 30 min before start of chemotherapy, w/ subsequent 4 mg given 4 & 8 hr thereafter. 4 mg tid may be given 1-2 days after the end of chemotherapy. Prevention & treatment of post-op nausea & vomiting Adult 8 or 16 mg 1 hr before anaesth followed by 2 further doses of 8 mg at 8-hr interval. Renal impairment Should not exceed 8 mg. Inj Prevention of PONV Adult 4 mg (undiluted) IV or IM as a single inj. Ped patients 1 mth-12 yr 4 mg single dose for patients weighing >40 kg. A single dose of 0.1 mg/kg for patients weighing <40 kg. Administration: The rate of administration should not be less than 30 sec over 2-5 min immediately prior to or following anesth induction, or post-op if patient did not receive prophylactic antiemetics & experiences nausea &/or vomiting occur shortly after surgery.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Concomitant use of apomorphine. Inj Pregnancy (in PONV) & lactation.
Special Precautions
Hypersensitivity to other selective 5-HT3 receptor antagonists; transient ECG changes including QT/QTc interval prolongation; may mask a progressive ileus & gastric distention. Patients w/ signs of subacute intestinal obstruction or ileus. Do not use to stimulate gastric or intestinal peristalsis. Hepatic & renal impairment. Pregnancy. Should not be used during lactation. Monitor childn >4 to 24 mth old. Inj May mask progressive ileus & gastric distention. Should not be used instead of nasogastric suction.
Adverse Reactions
Headache. Tab: Sensation of flushing or warmth, hiccups, & constipation. Inj: Chemotherapy-induced nausea & vomiting: Diarrhea, fever. Post-op nausea & vomiting: Drowsiness/sedation, fever, cold sensation, pruritus, paresthesia.
Drug Interactions
Additional QT prolongation w/ QT prolonging drugs. Increased risk of arrhythmias w/ cardiotoxic drugs eg, anthracyclines (eg, doxorubicin, daunorubicin or trastuzumab), antibiotics (eg, erythromycin), antifungals (eg, ketoconazole), antiarrhythmics (eg, amiodarone) & β-blockers (eg, atenolol or timolol). Altered mental status, autonomic instability & neuromuscular abnormalities w/ SSRIs & SNRIs. Profound hypotension & loss of consciousness w/ apomorphine. Drugs that prolong the QT interval &/or cause electrolyte abnormalities. May reduce analgesic effect of tramadol. Tab: Increased oral clearance & decreased blood conc w/ potent inducers of CYP3A4 (ie, phenytoin, carbamazepine, & rifampicin).
