Indications/Uses
Resp tract infections eg, pneumonia, bronchitis; ear, nose, throat, renal & UTI, genital infection eg, gonorrhea; sepsis; skin, wounds & soft tissue infections; peritonitis, cholecystitis, cholangitis, infections of GIT, infections in patients w/ immunodeficiency; meningitis.
Dosage/Direction for Use
Adult & childn >12 yr 1-2 g IV/IM once daily. Severe cases or infections caused by moderately sensitive organisms May be increased to 4 g once daily. Infant & childn 15 days-12 yr 20-80 mg/kg once daily. Childn ≥50 kg Usual adult dose. IV doses of ≥50 mg/kg should be given by IV infusion over at least 30 min. Neonate w/in 14 days 20-50 mg/kg. Max: 50 mg/kg. Bacterial meningitis Infant & childn Initially 100 mg/kg once daily. Max: 4 g once daily. Gonorrhea 250 mg single IM dose. Peri-op prophylaxis 1-2 g as single dose 30-90 min prior to surgery.
Contraindications
History of shock to ceftriaxone or hypersensitivity to penicillin, other cephalosporins & local anesth of anilides eg, lidocaine (IM only).
Special Precautions
History of drug allergy, patients whose relatives are prone to suffer from allergic symptoms eg, bronchial asthma, exanthema & urticaria. Severe renal disorder; poor oral ingestion or parenteral nutrition & elderly w/ poor general conditions. Cautiously monitor for vit K deficiency. Determine susceptibility & continue treatment for a minimum period of time. Perform skin reaction (shock monitoring) & lab test (hepatic & renal function, blood) at regular intervals. Shadows or precipitates (mistaken as gallstones) may be detected on sonograms. Discontinue use if hypersensitivity reactions occur. False +ve reaction for Benedict's & Fehling's test; galactosemia. +ve result to direct Coombs' test. Pregnancy & lactation. Neonates & premature babies.
Adverse Reactions
Hypersensitivity reactions eg, exanthema, urticaria, erythema, flare, pruritus, shivering, fever, allergic dermatitis, edema, erythema multiforme, anaphylactic or anaphylactic-like reaction; granulocytopenia, eosinophilia, thrombocytosis, leukopenia; elevation of AST, ALT, ALP & symptomatic precipitation of Ca salt in the gall bladder; headache.
Drug Interactions
May worsen renal insufficiency w/ similar compd (other cephem antibiotics) & diuretics eg, furosemide. Observe synergistic effect w/ aminoglycosides & antagonistic effect w/ chloramphenicol.
