Indications/Uses

Conditions where inhibition of gastric acid secretion may be beneficial, including aspiration syndromes, dyspepsia, GERD, PUD & Zollinger-Ellison syndrome.

Dosage/Direction for Use

Administer IV inj over 5 min or IV infusion over 20-30 min. Prophylaxis of acid aspiration 40 mg completed 1 hr before surgery. Gastric (benign) & duodenal ulcer, GERD 40 mg once daily until PO is possible. Severe peptic ulcer bleeding Initially 80 mg IV infusion then by continuous IV infusion 8 mg/hr for 72 hr (then change to oral therapy).

Contraindications

Hypersensitivity.

Special Precautions

Consider possibility of malignancy in patients w/ gastric ulcers since PPIs may mask symptoms & delay diagnosis. Hepatic impairment. Childn.

Adverse Reactions

Headache, diarrhea, & skin rashes. Pruritus, dizziness, fatigue, constipations, nausea & vomiting, flatulence, abdominal pain, arthralgia & myalgia, urticaria & dry mouth. Isolated cases of photosensitivity, bullous eruption, erythema multiforme, angioedema, & anaphylaxis. CNS effects including occasional insomnia, somnolence, & vertigo; reversible confusional states, agitation, depression, & hallucinations in severely ill patients. Raised liver enzymes, & isolated cases of hepatitis, jaundice & hepatic encephalopathy.

Drug Interactions

May alter metabolism of some drugs metabolized by cytochromes P450 system primarily by isoenzyme CYP2C19. May prolong elimination of diazepam, phenytoin, & warfarin. May reduce absorption of ketoconazole, & possibly itraconazole. Plasma conc of both drugs may be increased w/ voriconazole.
